基于OpenEHR的体检ECG等数据接口的研究与初步应用

【摘要】:随着医疗信息化的深入发展,医疗信息系统间的数据隔阂和信息孤岛问题日益突出,医疗信息共享和交换的需求越来越迫切。而医疗数据共享的标准不统一是导致异构信息系统间的数据交换困难的主要原因,特别是个人难以获取自身的健康档案,严重影响就医和社会健康服务。本文在分析HL7CDA标准和OpenEHR规范的基础上,通过构建开放及开源的相关健康档案,探索方便个人获取在各个医疗及健康服务机构的个人健康信息的新途径。本文主要针对健康体检信息特别是心电图(ECG)和尿检数据,分别利用HL7CDA标准构建CDA文档和OpenEHR规范构建原型文件,并提出了一种基于MongoDB的OpenEHR数据存储方案。能够针对心电和尿检数据进行手动录入,Excel数据文档导入等方式接入,通过OpenEHR格式数据转换处理,支持OpenEHR格式共享文件的生成和解析功能,实现了移动电子健康档案的初步构想。在此基础上,设计并实现了具有相关健康数据采集和管理以及健康干预服务的Web系统,详细分析了健康干预的相关业务流程和具体实现,实现了基于OpenEHR体检数据接口的初步应用。系统测试和运行结果表明已基本达到了预期的数据处理和业务要求,对健康服务的发展和构建区域性医疗信息共享平台具有一定的参考价值。
[Abstract]:With the development of medical information, the problem of data gap and information island between medical information systems is becoming more and more serious, and the need of medical information sharing and exchanging is becoming more and more urgent. However, the inconsistent standard of medical data sharing is the main reason for the difficulty of data exchange among heterogeneous information systems, especially the difficulty for individuals to obtain their own health records, which seriously affects access to medical and social health services. Based on the analysis of HL7CDA standards and OpenEHR specifications, this paper explores a new way to facilitate individuals' access to personal health information in various medical and health services by constructing open and open source health records. Aiming at the health examination information, especially ECG data and urine test data, this paper uses HL7CDA standard to construct CDA document and OpenEHR specification to build prototype file, and proposes a OpenEHR data storage scheme based on MongoDB. It can input the ECG and urine data manually into the Excel data file, and support the generation and analysis of the shared files in the OpenEHR format through the OpenEHR format data conversion processing. The preliminary conception of mobile electronic health file is realized. On this basis, the Web system with relevant health data collection and management and health intervention service is designed and implemented, and the related business process and implementation of health intervention are analyzed in detail. The primary application of physical examination data interface based on OpenEHR is realized. The system test and running results show that the expected data processing and business requirements have been basically met, which has a certain reference value for the development of health services and the construction of regional medical information sharing platform.
