【摘要】:目的:研究幽门螺杆菌(Helicobacter pylori,Hp)感染对胃黏膜DUOX2和NOX2基因和蛋白表达的影响,进一步揭示胃疾病的发病机理。方法:收集2013年8月至2014年2月有上消化道症状到河南科技大学第一附属医院做胃镜检查的患者胃黏膜组织标本200例,其中男性113例,女性87例,年龄16-86岁,平均52.3岁。采用快速尿素酶试验检测Hp感染情况;酶联免疫吸附实验检测Hp阳性患者血清中Cag A和Vac A抗体的表达情况;RT-PCR检测胃黏膜DUOX2和NOX2 m RNA的表达情况;免疫组化和Western Blot检测胃黏膜DUOX2和NOX2蛋白的表达情况。结果:1.快速尿素酶试验结果显示Hp阳性者111例(男69例,女42例),Hp阴性者89例(男44例,女45例)。2.RT-PCR结果显示DUOX2 m RNA在Hp阳性患者胃黏膜组织中的表达明显低于Hp阴性患者(P=0.02);Hp毒力强(Cag A+/Vac A+)的菌株胃黏膜DUOX2 m RNA表达量低,Hp毒力弱(Cag A-/Vac A-)的菌株胃黏膜DUOX2m RNA表达量高(均P0.05)。胃黏膜NOX2 m RNA水平在Hp(+)组的表达量高于Hp(-)组(P0.05),但在感染不同毒力的Hp的组别之间NOX2m RNA表达无明显差别。3.免疫组化结果显示DUOX2蛋白在200例胃黏膜组织中均有表达,与基因水平上的表达一致。DUOX2蛋白在Hp阳性组患者胃黏膜表达量明显低于Hp阴性组(P0.05);在Hp阳性的胃黏膜组织中与Cag A-/Vac A-组相比,Cag A+或Vac A+组DUOX2蛋白表达降低,Cag A+/Vac A+组DUOX2蛋白表达最低(均P0.05)。NOX2表达于胃黏膜炎性细胞内,Hp(+)组NOX2蛋白的表达较Hp(-)组升高(P0.05);在感染不同毒力的Hp的组别之间NOX2蛋白表达无差别。4.Western Blot结果显示胃黏膜组织DUOX2蛋白在Hp(+)组的表达量低于Hp(-)组,后者是前者的2.2倍。结论:1.Hp感染后胃黏膜DUOX2表达降低,NOX2表达升高,提示Hp感染对胃黏膜DUOX2和NOX2的表达均具有一定的影响作用。2.Hp感染后抑制了DUOX2的表达,且毒力越强的菌株对DUOX2表达的抑制作用越强;相反,Hp的感染促进了胃黏膜NOX2的表达,但NOX2表达量的多少与Hp的毒力无关。3.胃黏膜DUOX2和NOX2在Hp感染所致胃疾病的发生发展中起到一定的作用。
[Abstract]:Objective: To study the effect of Helicobacter pylori (Helicobacter pylori, Hp) infection on the expression of DUOX2 in gastric mucosa and NOX2 gene and protein, further reveal the pathogenesis of gastric diseases. Methods: from August 2013 to February 2014 with upper gastrointestinal symptoms in Henan University of Science and Technology First Affiliated Hospital patients for gastroscopy gastric mucosa specimens from 200 patients, among them 113 cases were male, 87 were female, aged 16-86 years old, average 52.3 years old. By rapid urease test to detect Hp infection; the expression of enzyme-linked immunosorbent assay in patients with positive serum Hp Cag A and Vac A antibody; expression of RT-PCR DUOX2 and NOX2 M detection of gastric mucosa RNA expression; immunohistochemistry Western and Blot detection of DUOX2 in gastric mucosa and NOX2 protein. Results: 1. rapid urease test results showed that 111 cases were Hp positive (male 69 cases, female 42 cases), 89 cases of Hp negative patients (male 44 cases, female 45 cases).2.RT -PCR results showed that the expression of DUOX2 m RNA in Hp positive patients with gastric mucosa was significantly lower than that of Hp negative patients (P=0.02); Hp (Cag A+/Vac A+) virulent strain DUOX2 in gastric mucosa of M RNA low expression, Hp (Cag A-/Vac A-) weak virulent strains of DUOX2m in gastric mucosa of RNA high expression (P0.05 NOX2 m). Gastric mucosal RNA levels in Hp (+) expression group was higher than that of Hp (-) group (P0.05), but in between the infection of different Hp strains group NOX2m RNA expression had no significant difference.3. immunohistochemistry showed that DUOX2 protein in 200 cases of gastric mucosa tissues. The expression of.DUOX2 protein the expression of Hp in gastric mucosa of patients with positive group were significantly lower than that in Hp negative group and gene expression level of (P0.05); in Hp positive gastric mucosa compared with Cag A-/Vac group A-, Cag A+ or Vac A+ reduced the expression of DUOX2 protein in Cag group, A+/Vac A+ group of DUOX2 protein expression in the lowest expression of.NOX2 (P0.05) In the gastric mucosa inflammatory cells, Hp (+) NOX2 protein expression group than in Hp (-) group (P0.05); infection between different Hp strains group NOX2 protein expression.4.Western Blot showed no difference in gastric mucosa of DUOX2 protein in Hp (+) expression was lower than that in Hp (- group), which is 2.2 times the former. Conclusion: 1.Hp infection in gastric mucosa after DUOX2 expression decreased, NOX2 expression increased, suggesting that Hp infection on the expression of DUOX2 in gastric mucosa and NOX2 have certain influence after.2.Hp infection inhibited the expression of DUOX2, the stronger the inhibition and the more virulent strain on the expression of DUOX2 on the contrary, Hp infection; promote the expression of NOX2 in gastric mucosa, but the expression of NOX2 and the amount of Hp is independent of.3. DUOX2 in gastric mucosa and toxicity of NOX2 in Hp induced gastric diseases play a certain role in the development.
