血清肿瘤坏死因子相关凋亡诱导配体与C-反应蛋白对儿童急性病毒感染的诊断价值

【摘要】:目的探讨急性细菌和病毒感染患儿血清肿瘤坏死因子相关凋亡诱导配体(TRAIL)的水平变化及临床价值。方法选取2013年1月-2016年4月医院住院的急性病毒感染患儿51例、急性细菌感染患儿52例为研究对象,对照组选取同期医院体检未发现感染疾病者51例,采用酶联免疫吸附法检测血浆TRAIL水平,西门子BNⅡ特定蛋白分析仪测定血清C-反应蛋白(CRP)。结果不同组别中TRAIL和CRP水平明显不同(P0.05),病毒感染组TRAIL水平明显高于细菌感染组和正常对照组(P0.05),病毒感染组CRP水平明显低于细菌感染组,但高于正常对照组(P0.05);受试者工作特征曲线分析,TRAIL和CRP在区分急性病毒感染患者和正常对照者的曲线下面积分别为0.759和0.744,以147.35pg/mL和5.415g/L为诊断界值点时,灵敏度分别为76.0%、70.0%,特异度分别为76.5%、88.2%;TRAIL与CRP联合检测,灵敏度和特异度分别提高到78.0%和92.2%。结论高水平血清TRAIL可能对患儿发生急性病毒感染有一定的预测价值,TRAIL和CRP联合检测提高急性病毒感染诊断效能。
[Abstract]:Objective to investigate the changes and clinical value of serum tumor necrosis factor-related apoptosis-inducing ligand trail in children with acute bacterial and viral infection. Methods 51 hospitalized children with acute viral infection from January 2013 to April 2016 were selected. 52 children with acute bacterial infection were studied. 51 patients with no infection were selected as control group. Plasma TRAIL levels were detected by enzyme linked immunosorbent assay (Elisa). Results the levels of TRAIL and CRP were significantly different in different groups. The TRAIL level in viral infection group was significantly higher than that in bacterial infection group and normal control group (P 0.05). The CRP water in viral infection group was significantly higher than that in bacterial infection group and normal control group. The level of ping was significantly lower than that of bacterial infection group. The area under the curve of trail and CRP were 0.759 and 0.744, 147.35pg / mL and 5.415g / L, respectively. The sensitivity was 70.0 and the specificity was 76.588. 2%, respectively. Trail was detected with CRP, and the sensitivity was 76. 0%, and the specificity was 76. 5% and 88. 2%, respectively. Conclusion the high level of serum TRAIL may be a predictor of acute viral infection in children. The combined detection of trail and CRP can improve the diagnostic efficacy of acute viral infection.
