2011-2014年金黄色葡萄球菌分布与耐药变化趋势研究

【摘要】:目的分析医院2011-2014年金黄色葡萄球菌(SAU)分布及耐药性,为抗菌药物合理选用提供参考。方法收集医院2011年1月-2014年12月临床分离的SAU,并使用MicroScan WalkAway96plus全自动微生物分析仪进行鉴定及药敏试验,采用WHONET5.5软件对病原菌资料进行回顾性分析。结果共分离SAU913株,其中耐甲氧西林金黄色葡萄球菌(MRSA)407株,检出率为44.58%,MRSA的检出率自2011年的45.65%降至2014年的38.03%,呈显著下降趋势;SAU主要分布于骨科287株占31.44%、神经外科203株占22.23%、烧伤科118株占12.92%和ICU 105株占11.50%,其中烧伤科MRSA的检出率连续4年均高于73.10%,明显高于其他病区;各类临床标本中分泌物标本中分离的SAU最高467株,占51.15%,其次痰标本335株,占36.69%;MRSA对常用的抗菌药物均有较高的耐药率,但对庆大霉素、环丙沙星、左氧氟沙星的耐药率呈明显下降趋势。结论 SAU的临床分离率呈上升趋势,应加强对其耐药性监测,以指导临床合理用药,并有效控制MRSA的医院感染。
[Abstract]:Objective to analyze the distribution and drug resistance of Staphylococcus aureus (Staphylococcus aureus) in hospital from 2011 to 2014. Methods the clinical isolates of SAU from January 2011 to December 2014 in hospital were collected, and MicroScan WalkAway96plus automatic microbiological analyzer was used for identification and drug sensitivity test. The data of pathogenic bacteria were analyzed retrospectively by WHONET5.5 software. Results A total of 407 strains of SAU913 were isolated, including 407 strains of methicillin-resistant Staphylococcus aureus. The detectable rate of MRSA decreased from 45.65% on 2011 to 38.03 on 2014. The positive rate of sau mainly distributed in orthopedics 287strains (31.4444), neurosurgery 203strains (22.23234d), burn department 118strains (12.92% strains) and ICU 105strains (11.50g). The detection rate of MRSA in burn department was 11.50%. For 4 consecutive years, it was higher than 73.10, which was obviously higher than that in other areas. There were 467 strains of SAU isolated from all kinds of clinical specimens, accounting for 51.15 strains, followed by 335 strains of sputum, accounting for 36.69% of MRSA, but they were resistant to gentamicin and ciprofloxacin. Conclusion the clinical isolation rate of SAU is on the rise, so it is necessary to strengthen the monitoring of drug resistance in order to guide rational drug use in clinic and to effectively control nosocomial infection of MRSA.
