深圳地区无偿献血HBsAg阳性感染者的血清学和分子生物学特征分析

【摘要】:目的了解深圳市无偿献血者乙肝病毒感染情况,分析其血清学和分子生物学特征。方法将收集到的确证HBs Ag(+)无偿献血者标本分为/NAT(+)和NAT(-)两组,进行乙肝两对半检测以及病毒核酸大容量提取,采用巢氏PCR方法扩增BCP/PC和S区基因序列,对所得的序列进行分析。结果在42 297份血液中235份HBs Ag确证为阳性的标本,HBs Ag阳性率为0.56%,其中NAT(+)162份,占总数68.9%,NAT(-)为73份,占31.1%,血清学模式以HBs Ag(+)、抗-HBe(+)、抗-HBc(+)为主,占69.78%,检出10例抗-HBc(-)标本,占总数的4.25%。在可测序166标本中,B基因型所占88%(146/166),C基因型占11.4%(19/166),D基因型在HBs Ag(+)/NAT(+)出现1例。结论 NAT检测与HBs Ag检测方法均只能检出部分乙肝感染献血者,血液检测必须结合两种检测方法来保证血液安全。数据显示有必要应用灵敏度更高的NAT检测方法。
[Abstract]:Objective to investigate hepatitis B virus infection in blood donors in Shenzhen City and analyze its serological and molecular biological characteristics. Methods the samples of confirmed HBs Ag() donors were divided into two groups: nat () and NAT-). The HBV DNA was detected in two pairs and the virus nucleic acid was extracted in large volume. The nucleotide sequences of BCP/PC and S region were amplified by nested PCR method. The sequence was analyzed. Results the positive rate of HBs Ag in 235 of 42,297 blood samples confirmed to be positive for HBs Ag was 0.56. The positive rate of Nat (162 cases) was 73 (31.1%). The serological model was HBs AgA (HBs, anti-HBec, anti-HBcn (69.78g). Accounting for 4.25% of the total. Of the 166 samples sequenced, 88% of the alleles were genotype B (146R / 166C) and one case was found in HBs (Agna / NATA () with 11.4% / 166C / 166C genotype. Conclusion both NAT and HBs Ag detection methods can only detect some blood donors infected with hepatitis B, blood detection must be combined with two methods to ensure blood safety. The data show that it is necessary to apply a more sensitive NAT detection method.
