偏瘫患者不同坐姿下腹肌厚度变化的研究

【摘要】:第一部分:健康人两侧腹肌在不同坐姿下的厚度变化的研究目的运用超声测量技术,比较正常人两侧腹肌在不同稳定度的坐姿下腹横肌和腹内斜肌厚度的变化。方法招募正常受试者(N=30)参与此次研究。通过超声测量三种不同坐位(坐无靠背椅、坐靠背椅及坐体操球)平衡状态下两侧的肌肉厚度与仰卧休息体位下测得的腹横肌和腹内斜肌的肌肉厚度的变化(三种不同坐姿肌肉厚度变化/仰卧位肌肉厚度),然后将得出的结果以百分数的形式表示肌肉厚度的变化。结果正常受试者腹横肌和腹内斜肌的厚度改变百分比随着坐姿的稳定度的下降而增加,左右两侧腹内斜肌与腹横肌肌肉厚度皆无显著差异(P0.05)。结论腹横肌及腹内斜肌对于正常人坐位平衡的调节具有重要作用,且正常人两侧腹肌是对称平衡的。第二部分:偏瘫患者不同坐姿下腹肌厚度变化的研究目的运用超声测量技术,比较偏瘫患者与正常对照组在不同稳定度的坐姿下腹横肌和腹内斜肌厚度的变化。方法招募年龄、身高、体重相匹配的偏瘫患者(N=30)和正常受试者(N=30)参与此次研究。通过超声测量三种不同坐位(坐无靠背椅、坐靠背椅及坐体操球)平衡状态下肌肉厚度与仰卧休息体位下测得的腹横肌和腹内斜肌的肌肉厚度的变化(三种不同坐姿肌肉厚度变化/仰卧位肌肉厚度),然后将得出的结果以百分数的形式表示肌肉厚度的变化。结果偏瘫患者腹横肌的厚度改变百分比随着坐姿的稳定度的下降而变薄,但是腹内斜肌无明显变化,而正常受试者腹横肌和腹内斜肌的厚度改变百分比随着坐姿的稳定度的下降而增加。在正常人和偏瘫患者之间,坐在体操球上或坐在椅子上(无论有无靠背),正常受试者和偏瘫患者腹横肌变化有显著差距。而在不同坐姿时,两组内腹内斜肌厚度变化无明显差距。结论随着坐位稳定度的下降,正常受试者及偏瘫患者腹横肌厚度变化是相反的,正常受试者逐渐增大,偏瘫患者逐渐减小。
[Abstract]:Part one: study on the thickness changes of bilateral abdominal muscles in healthy subjects under different sitting positions objective to compare the thickness of the inferior transverse and oblique abdominal muscles in the normal subjects with different stability by using ultrasonic measurement technique. Methods normal subjects (NN30) were recruited to participate in the study. Measured by ultrasound in three different seating positions (sitting in a backless chair, Muscle thickness of both sides in balanced position and that of abdominal transverse muscle and internal oblique muscle measured in supine resting position (three different sitting positions / supine position) The result is then expressed as a percentage change in muscle thickness. Results the percentage of the thickness changes of the transverse and oblique abdominal muscles increased with the decrease of sitting position in normal subjects, and there was no significant difference between the left and right oblique abdominal muscles and the transverse abdominal muscles in the left and right sides (P0.05). Conclusion the abdominal transverse muscle and the internal oblique muscle play an important role in regulating the balance of sitting position in normal people, and the bilateral abdominal muscles of normal people are symmetrical balanced. Part two: study on the thickness change of abdominal muscle in different sitting posture of hemiplegic patients objective to compare the thickness of abdominal transverse muscle and internal oblique muscle between patients with hemiplegia and normal control group. Methods to recruit age, height and weight matched hemiplegic patients (NN30) and normal subjects (N30) to participate in the study. Measured by ultrasound in three different seating positions (sitting in a backless chair, The changes in muscle thickness in balance between the abdominal transverse muscle and the internal oblique muscle measured under the supine resting position (the muscle thickness changes in three different sitting positions / in the supine position), and then the muscle thickness in the supine position and the muscle thickness of the abdominal transverse muscle and the internal oblique muscle in the supine position are measured under the balance of the sitting chair and the gymnastic ball. The result is expressed as a percentage change in muscle thickness. Results the percentage of the thickness change of abdominal transverse muscle in hemiplegic patients decreased with the stability of sitting position, but there was no obvious change in the oblique abdominal muscle. The percentage change of the thickness of the transverse and oblique abdominal muscles in normal subjects increased with the decrease of the stability of sitting position. There was a significant difference between normal subjects and hemiplegic patients in the changes of abdominal transverse muscles between normal subjects and patients with hemiplegia, sitting on a gymnastic ball or sitting on a chair (with or without backrest). However, there was no significant difference in the thickness of the internal oblique abdominal muscle between the two groups in different sitting positions. Conclusion the thickness of abdominal transverse muscle in normal subjects and hemiplegic patients is opposite with the decrease of sitting stability. The thickness of abdominal transverse muscle increases gradually in normal subjects and decreases in patients with hemiplegia.
