催眠镇痛的临床应用及神经生物学机制

【摘要】:目的总结催眠镇痛的临床应用及相关神经生物学机制研究,为进一步探索催眠镇痛的生物学机制指明方向。方法在Pubmed、Elsevier、Springerlink、google学术等数据库中,以"hypno*" 、"pain" 、"Neuroimaging" 、"electroencephalogram(EEG)"等为关键词查询相关文献,对相关文献进行综述。结果催眠能有效缓解急慢性疼痛,减轻疼痛伴随的躯体不适及不良情绪。临床上催眠被广泛用于手术或临床有创操作的镇痛及治疗癌性疼痛,近些年来也被用于也被用于放射介入过程中的镇痛。其神经影像学研究发现催眠镇痛可能与疼痛相关脑区和扣带回的调节功能有关,脑电图研究显示催眠镇痛与注意力高度集中、情绪处理、认知改善有关。结论催眠镇痛的神经生物学机制研究国外已做出初步探索,其临床应用在国内需进一步推广,其作用机制目前仍缺乏一个系统的理论,有待进一步探索。
[Abstract]:Objective to summarize the clinical application and neurobiological mechanism of hypnotic analgesia, and point out the direction of further exploring the biological mechanism of hypnotic analgesia. Methods in Pubmedus Elsevier Spring link Google academic database, "hypnol *", "pain", "Neuroimaging" and "electroencephalogram" were selected as keywords to review the relevant literatures. Results hypnosis can effectively relieve acute and chronic pain, pain associated with physical discomfort and bad mood. Hypnosis is widely used in surgical or clinical invasive analgesia and in the treatment of cancerous pain. In recent years, hypnosis has also been used in radio-interventional analgesia. The neuroimaging study found that hypnotic analgesia may be related to the regulatory function of pain related brain area and cingulate gyrus. Electroencephalogram showed that hypnotic analgesia was related to high concentration of attention emotional processing and cognitive improvement. Conclusion the neurobiological mechanism of hypnotic analgesia has been preliminarily explored abroad, and its clinical application needs to be further promoted in China. The mechanism of hypnotic analgesia still lacks a systematic theory and needs further exploration.
