“以痛镇痛”:条件性疼痛调节

【摘要】:条件性疼痛调节(CPM)是一种内源性疼痛调节,指身体某一部位的持续性疼痛可以抑制施于身体另一部位的伤害性刺激所引起的疼痛感知.本文首先概述了与CPM相关的神经生理、生化机制,并介绍了测量个体CPM效率的传统心理物理学方法,总结了基于脑电和功能磁共振等神经成像技术评估CPM效率的神经生理指标,并提出应综合心理物理学和神经生理学方法评估个体CPM功能.然后,从人口统计学、生理学和心理学几个方面详细探讨了影响个体CPM功能的复杂因素,从而提示大家应综合考虑影响个体CPM效率的因素.最后,本文总结了CPM相关的临床应用,包括慢性疼痛预测与评估、镇痛药物剂量预测以及个体健康质量评估等.本文既能加深对人类疼痛调节功能潜在机制的理解,又能为临床上慢性疼痛疾病的预测和评估提供参考依据.
[Abstract]:Conditioned pain regulation (CPM) is an endogenous pain regulation, which indicates that persistent pain in one part of the body can inhibit pain perception caused by nociceptive stimuli applied to another part of the body. In this paper, the neurophysiological and biochemical mechanisms related to CPM are summarized, and the traditional psychophysical methods for measuring the efficiency of individual CPM are introduced, and the neurophysiological indexes for evaluating the efficiency of CPM based on EEG and functional magnetic resonance imaging techniques are summarized. It is suggested that the individual CPM function should be evaluated by the methods of psychophysics and neurophysiology. Then, from the aspects of demography, physiology and psychology, this paper discusses in detail the complex factors that affect the function of individual CPM, which suggests that we should consider the factors that affect the efficiency of individual CPM. Finally, the clinical application of CPM was summarized, including the prediction and evaluation of chronic pain, the dose prediction of analgesic drugs and the evaluation of individual health quality. This article can not only deepen the understanding of the potential mechanism of human pain regulation function, but also provide a reference for clinical prediction and evaluation of chronic pain diseases.
