简易促通装置对痉挛型偏瘫患儿手功能康复的作用

【摘要】:目的:研制简易手功能训练装置,并探讨此装置对痉挛型偏瘫患儿手功能康复的效果。方法:选取4—16岁痉挛型偏瘫患儿17例,按随机数表法分为实验组(9例)和对照组(8例)。试验组进行简易促通装置和常规康复训练,各1次/天;对照组进行常规康复训练2次/天。治疗前和治疗8周后,采用简易上肢功能评价(STEF)、改良Ashworth分级(MAS)、残疾儿童综合功能评定(ADL)及表面肌电图(s EMG)分别对两组患儿患手的运动功能、大鱼际肌群的肌张力、日常生活活动能力和大鱼际肌群的耐力进行评定。结果:治疗后,试验组STEF评分、ADL评分、MAS分级和患手大鱼际肌群表面肌电的均方根值(RMS)、平均频率(MEF)与治疗前比较,差异均具有显著性意义(P0.05);且两组间治疗后MEF比较,及治疗前、后MEF差值比较,差异具有显著性意义(P0.05)。结论:简易促通装置可降低痉挛型偏瘫患儿患手大鱼际肌群的肌张力,提高肌肉的耐力,提高运动功能和ADL能力。
[Abstract]:Objective: to develop a simple hand function training device and to explore the effect of hand function rehabilitation in children with spastic hemiplegia. Methods: 17 children aged 4-16 with spastic hemiplegia were randomly divided into experimental group (n = 9) and control group (n = 8). The experimental group was given simple facilitation device and routine rehabilitation training once a day, and the control group was treated with routine rehabilitation training twice a day. Before and after 8 weeks of treatment, simple upper limb function evaluation was used to evaluate the comprehensive function of (ADL) and surface electromyography (s EMG) in children with disabilities by (STEF), modified Ashworth grade (MAS),. The motor function of the affected hands and muscle tension of thenar muscles were evaluated by (ADL) and surface electromyography (s EMG), respectively. Activities of daily life and endurance of thenar muscles were evaluated. Results: after treatment, there were significant differences in STEF score and (RMS), mean root mean square (RMS) (MEF) between the two groups after treatment (P0.05), and between the two groups before and after the treatment, and before and after the treatment, there were significant differences between the two groups in the mean root mean square (RMS) value of the surface myoelectric activity of the hypothenar muscle group (P05), and before and after the treatment, there were significant differences between the two groups. The difference of post MEF difference was significant (P0.05). Conclusion: the simple facilitation device can reduce the muscle tension of the thenar muscles of the children with spastic hemiplegia, improve the endurance of the muscles, improve the motor function and the ability of ADL.
