急性白血病患儿父母心理弹性、社会支持与生活质量的相关性研究

【摘要】：目的了解急性白血病患儿父母心理弹性、社会支持、生活质量的现状,分析急性白血病患儿父母心理弹性、生活质量的影响因素,探讨急性白血病患儿父母的心理弹性、社会支持与生活质量三者之间的相关性。方法本研究属于描述性研究,采取便利抽样法,选取郑州大学第一附属医院小儿血液科(小儿内科三病区、小儿内科五病区)2014年11月-2015年1月期间住院的260名急性白血病患儿父母为研究对象,采用一般情况调查表、心理弹性量表(CD-RISC)、社会支持评定量表(SSRS)、生活质量测定量表(WHOQOL-BREF中文版)进行问卷调查。采用SPSS17.0统计软件对资料进行统计学分析。采用统计描述的方法分析患儿及父母的一般资料、心理弹性、社会支持、生活质量总分及各维度得分情况;采用方差分析、t检验等分析不同人口社会学资料对患儿父母心理弹性、生活质量的影响因素;采用相关分析,发现三者之间的相关性;通过多元逐步回归分析,确定影响患儿父母生活质量的主要因素。结果1.急性白血病患儿父母的心理弹性总得分为52.93±18.42分,其中坚韧纬度得分为27.58±10.43分,自强纬度得分为17.96±6.49分,乐观纬度得分为7.38±4.06分,以自强维度各条目得分最高,坚韧维度各条目得分最低。采用t检验及方差分析对不同人口社会学一般资料的心理弹性总得分进行比较,结果显示与患儿不同亲子关系、不同职业的患儿父母的心理弹性总得分差异有统计学意义(P0.05)。2.急性白血病患儿父母社会支持总得分为36.96±7.22分,其中客观支持得分5.46±0.46分,主观支持24.51±4.77分,对支持的利用度得分为6.97±1.64分,以客观支持维度各条目均分最高,支持利用度维度各条目均分最低。3.急性白血病患儿父母生活质量总得分为44.04±6.81分,其中生理领域得分为12.25±2.16分,心理领域得分为10.86.36±2.07分,社会关系领域得分为12.91±1.92分,环境领域得分为10.39±2.36分,采用t检验及方差分析对不同人口社会学一般资料的生活质量总得分进行比较,结果显示患儿疾病类型、与患儿不同亲子关系、付费方式三个方面的生活质量总得分差异有统计学意义(P0.05)。4.除心理领域与支持利用度维度、乐观维度与社会关系领域外,心理弹性、社会支持和生活质量及其其它各维度均呈正相关。5.以生活质量为因变量,社会支持、心理弹性各维度为自变量,多元逐步回归进行分析显示,影响生活质量的主要因素有坚韧、主观支持、支持利用度。结论1.急性白血病患儿父母的心理弹性、社会支持及生活质量均处于较低水平。心理弹性方面,影响心理弹性的人口社会学因素主要有与患儿不同亲子关系,不同职业。生活质量方面,影响生活质量的人口社会学因素主要有患儿疾病类型、与患儿关系及付费方式。2.患儿父母心理弹性及各维度、社会支持与生活质量除社会关系与乐观维度、心理领域与支持利用度维度外,其余各维度均呈正相关。3.社会支持中的主观支持维度、支持利用度和心理弹性的坚韧维度能很好的预测患儿父母生活质量水平。
[Abstract]:Objective To investigate the status of psychological elasticity, social support and quality of life in children with acute leukemia. There is a correlation between social support and quality of life. Methods A descriptive study was conducted to study the parents of 260 children with acute leukemia hospitalized from November 2014 to January 2015 from November 2014 to January 2015 in the First Affiliated Hospital of Zhengzhou University. A questionnaire was used for questionnaire, psychological elasticity scale (CD-RISC), social support scale (SSRS) and quality of life measurement (WHOQOL-BREF Chinese version). Statistical analysis of data was carried out using SPSS 10.0 statistical software. The general information, psychological elasticity, social support, life quality total score and each dimension score of the children and their parents were analyzed by means of statistical description. The factors influencing the psychological elasticity and quality of life of children were analyzed by means of variance analysis and t-test. Correlation analysis was used to find the correlation between them, and the main factors affecting the quality of life of parents were determined by stepwise regression analysis. Result 1. The psychological elasticity of parents in children with acute leukemia has to be divided into 52. 93 and 18. 42, among which the tough latitude is divided into 27. 58 and 10.43, the self-strengthening latitude is divided into 17. 96 and 6.49, and the optimistic latitude is divided into 7. 38 and 4.06. The score of each item in the self-improvement dimension is the highest, and the score of each item of the tenacity dimension is the lowest. Using t-test and variance analysis, the author compared the general psychological elasticity scores of the general information of different demographic sociology, and the results showed that there was a significant difference in the psychological elastic total scores of parents with different parentage and different occupations (P0.05). The social support of parents in children with acute leukemia has to be divided into 36. 96 and 7. 22 points, among which, the objective support score is 5. 46, 0. 46, the subjective support is 24. 51 and 4.77, and the utilization degree of support is divided into 6. 97 to 1.64, with the highest average distribution of each item in the objective support dimension, and the lowest... The quality of life of parents in children with acute leukemia has to be divided into 44. 04 to 6.81, among which the physiological fields are divided into 12. 25 and 2.16, and the psychological field is divided into 10.86. 36 and 2.07, and the social relations are divided into 12.91 and 1.92, and the environmental field is divided into 10.39 and 2.36, Using t-test and variance analysis to compare the general score of life quality of general information of different population sociology, the results showed that there was significant difference in the quality of life quality in the three aspects of children's disease type, parent-child relationship and payment method (P0.05). In addition to psychological field and support utilization dimension, there is positive correlation between optimistic dimension and social relation domain, psychological elasticity, social support and quality of life and other dimensions. The multivariate stepwise regression analysis showed that the quality of life was dependent on variable, social support and psychological elasticity, and the main factors influencing the quality of life were strong, subjective and supportive. Conclusion 1. The psychological elasticity, social support and quality of life of children with acute leukemia were lower. In the aspect of psychological elasticity, the social factors which influence the psychological elasticity mainly have different parentage and different occupation. In terms of quality of life, the demographic sociological factors affecting the quality of life mainly include the type of disease, the relationship between the children and the method of payment. Children's parents' psychological elasticity and dimensions, social support and life quality, besides social relations and optimistic dimension, psychological field and support utilization dimension, the remaining dimensions are positive correlation. 3. The subjective support dimension in social support, support the degree of utilization and the resilience of psychological elasticity can well predict the quality of life of the parents.
