唐山市农村基层医疗服务质量的调查与分析

【摘要】：伴着经济的快速发展以及人民生活水平的提高,我国社会的主要矛盾得到了有效的缓解,但是我国城乡差距显著,农村居民“看病贵、看病难”的现象仍然很严重。在新型农村合作医疗的背景下,农村基层医疗机构的地位和作用有了很大程度的提高,农村卫生事业水平得到整体改善,但是,县、乡、村三级医疗服务网络所承担的责任大相径庭,县级医疗机构主要负责重病急病的治疗,乡镇医疗机构以一些多发病、常见病的治疗为主,一般疾病常以村级医疗机构负责。因此,农村基层医疗机构的服务质量问题直接关系到农村居民的生活健康水平,研究农村居民对基层医疗机构的满意度问题,对改善农村基层医疗机构的服务质量,提高其地位和作用具有重要的现实意义。本文根据国内外医疗机构的研究现状,结合河北省唐山市农村基层医疗机构的特点,归纳总结出影响农村基层医疗机构服务质量的四个主要因素:基层医疗机构本身、医务人员、药品的使用以及医疗保障制度。按照这四个主要的影响因素设计出调查问卷,通过抽样调查获得河北省唐山市农村居民对基层医疗机构满意度情况的第一手数据。利用基本的描述性统计分析和数据挖掘的方法,对农村居民的基本情况和就医倾向进行分析,并建立结构方程模型,找出农村基层医疗服务质量的影响因素及其影响程度。结果表明,农村居民更加重视村卫生室的诊疗效果和医务人员技术水平的提高,而对村卫生室的硬件设施要求不高;要想提高乡镇卫生院的服务质量,必须全面协调发展软硬件设施,特别是医务人员整体水平的提高。根据农村居民对村卫生室和乡镇卫生院提出的具体的要求,本文从医疗机构本身、医务人员、药品的使用以及医疗保障制度四个方面提出相应的对策和建议。
[Abstract]:With the rapid development of economy and the improvement of people's living standard, the main contradiction of our society has been effectively alleviated, but the gap between urban and rural areas in our country is remarkable, and the phenomenon of "seeing a doctor is expensive and difficult" for rural residents is still very serious. In the context of the new rural cooperative medical system, the status and role of rural primary medical institutions have been greatly improved, the level of rural health services has been improved overall, but, county, township, The responsibility of village tertiary medical service network is quite different. County medical institutions are mainly responsible for the treatment of serious and acute diseases, township medical institutions are mainly responsible for the treatment of some frequent diseases and common diseases, and the general diseases are often taken care of by village medical institutions. Therefore, the service quality of rural primary medical institutions is directly related to the living and health level of rural residents. It is of great practical significance to improve its status and function. According to the current research situation of medical institutions at home and abroad and the characteristics of rural primary medical institutions in Tangshan City, Hebei Province, this paper sums up four main factors that affect the service quality of rural primary medical institutions: the primary medical institutions themselves. Medical personnel, drug use and medical insurance system. According to the four main influencing factors, the questionnaire was designed, and the first-hand data of satisfaction of rural residents to primary medical institutions in Tangshan City, Hebei Province were obtained by sampling survey. By using the basic descriptive statistical analysis and data mining methods, this paper analyzes the basic situation and the tendency to seek medical treatment of rural residents, and establishes a structural equation model to find out the influencing factors and the degree of influence on the quality of primary medical service in rural areas. The results show that the rural residents pay more attention to the diagnosis and treatment effect of the village clinics and the improvement of the technical level of the medical personnel, but not to the hardware facilities of the village clinics, and to improve the service quality of the township hospitals, The overall development of hardware and software facilities, especially the improvement of the overall level of medical personnel, must be coordinated. According to the specific requirements of rural residents to village clinics and township hospitals, this paper puts forward corresponding countermeasures and suggestions from four aspects: medical institution itself, medical staff, drug use and medical security system.
