双相情感障碍患者静息态功能磁共振Meta分析

【摘要】:目的:双相情感障碍(Bipolar disorder,BD)是一种高致残、高疾病负担的重性精神障碍,静息态下低频振幅(Amplitude of low frequency fluctuation,ALFF)以及局部一致性(Regional homogeneity,ReHo)异常提示双相情感障患者脑功能活动异常。本研究系统评价双相情感障碍患者与健康对照组静息态功能磁共振低频振幅及局部一致性的差异,旨在探索双相情感障碍患者神经影像学标志物。方法:计算机检索Pubmed、Embase、Ovid、Web of Science、EBSCOhost medline、中国生物医学文献数据库(CBMdisc)、CNKI、VIP以及万方数据库,收集关于双相情感障碍患者及健康对照组静息态功能磁共振ALFF及ReHo的病例对照研究。按系统评价方法,由两名研究者独立进行质量评价和资料提取,采用Anisotropic Effect-Size-Based Algorithms(AES-SDM)(内置P0.005,体素k=10)进行坐标预处理、异质性检验、敏感度分析、阈值检验、元回归分析,得出差异脑区生成结果图像。结果:本研究共纳入8篇文献,其中ALFF相关文献4篇,样本量为206例,其中病例组及健康对照组各103例。Meta分析研究结果显示,与健康对照相比,病例组ALFF值增高的脑区有:左侧豆状核,左前丘脑,右侧额下回;ALFF值降低的脑区有:左侧舌回,左侧小脑区域(半球小叶Ⅰ,IV/V,梭回)。ReHo相关文献4篇,样本量为155例,其中病例组75例,健康对照相80例。Meta分析研究结果显示,与健康对照相比,病例组ReHo值增高的脑区有:右侧杏仁核,右侧海马,左尾状核;ReHo值降低的脑区有:右侧颞上回,左侧前额回,额内侧回。结论:双相情感障碍患者在左侧基底核区域、右侧额下回、左侧舌回及左侧小脑区域存在ALFF值异常,在右侧杏仁核及海马区域,左尾状核、右侧颞上回、左侧前额回、额内侧回存在ReHo值异常,提示双相情感障碍的发病可能与这些脑区局部功能异常有关。
[Abstract]:Objective: bipolar disorder (BDD) is a kind of mental disorder with high disability and high burden of disease. Low frequency amplitude of low frequency fluctuation. ALF) and regional homogeneity. This study systematically evaluated the differences of resting FMR low frequency amplitude and local consistency between bipolar affective disorder patients and healthy controls. Objective to explore neuroimaging markers in patients with bipolar affective disorder (bipolar disorder). Methods: Pubmedmedus Embase Ovidof Science was searched by computer. EBSCOhost Medline, Chinese Biomedical Literature Database (CBM) and Chinese Biomedical Literature Database (EBSCOhost) and Wanfang database. A case-control study on resting functional magnetic resonance (ALFF) and ReHo in patients with bipolar disorder and healthy controls was conducted. The quality evaluation and data extraction were carried out by two researchers independently. Anisotropic Effect-Size-Based algorithm AES-SDM (built-in P0.005) is used. Coordinate preprocessing, heterogeneity test, sensitivity analysis, threshold test and meta-regression analysis were performed to obtain the resulting image of differential brain region. Results: eight articles were included in this study. Among them, there were 4 articles related to ALFF, the sample size was 206 cases, among them, the case group and the healthy control group were 103 cases each. Meta-analysis results showed that compared with the healthy control group. In the case group, the ALFF increased in the left lentiform nucleus, the left anterior thalamus and the right inferior frontal gyrus. The ALFF decreased in the left lingual gyrus and the left cerebellar area (I / V in the hemispheric lobules I / V, 4 articles in the gyrus fusifolia. ReHo). The sample size was 155 cases, including 75 cases in the case group. The results of meta analysis of 80 cases of healthy radiographs showed that the ReHo increased in the right amygdaloid nucleus, the right hippocampus and the left caudate nucleus in the case group compared with the healthy control group. ReHo decreased in the right superior temporal gyrus, left prefrontal gyrus, medial frontal gyrus. Conclusion: bipolar affective disorder patients in the left basal nucleus region, right inferior frontal gyrus. In the right amygdala and hippocampus, the left caudate nucleus, the right superior temporal gyrus, the left prefrontal gyrus and the medial frontal gyrus were abnormal. The results suggest that the pathogenesis of bipolar disorder may be related to the abnormal local function of these brain regions.
