膝关节手术患者临床用血分析

【摘要】：目的了解本院骨科膝关节手术用血情况,为临床合理用血提供依据。方法回顾性分析2015年上半年在本院行膝关节手术接受输血的181例患者的临床资料。分析成分血及自体血使用情况,及对输血相关因素如性别、血型、年龄、凝血指标和住院天数等进行统计学分析。结果 181例患者女性较多为141例,AB型患者最少,不同性别和血型与平均输血量比较,差异均无统计学意义(P0.05)。平均输血量与手术类型和年龄有显著相关性(P0.05),双膝置换较单膝手术用血量大,50-70岁年龄段平均输血量最大。备自体血患者输血后RBC、Hb和Hct值较输血前有所下降但均在正常值范围内。未输血浆患者术后凝血指标均正常,随着输血总量的增加,平均住院天数增多。结论年龄和手术类型与平均输血量有明显的相关性,我们应严格把握输血适应症,选择安全节约的用血方式。
[Abstract]:Objective to investigate the blood use of knee joint surgery in orthopedic department of our hospital. Methods the clinical data of 181 patients who received blood transfusion during knee joint operation in our hospital in 2015 were analyzed retrospectively. The use of blood components and autologous blood was analyzed. Blood transfusion related factors such as sex, blood type, age, coagulation index and hospitalization days were statistically analyzed. There was no significant difference between gender, blood type and average blood transfusion volume. There was a significant correlation between average blood transfusion volume and operation type and age (P 0.05). The average blood volume of knee replacement was larger than that of single knee operation. The average blood transfusion volume of patients aged 50-70 years was the largest. RBC was obtained in patients with autologous blood transfusion. The HB and Hct values were lower than those before blood transfusion, but were within the normal range. The blood coagulation indexes were normal in the patients without plasma transfusion, and with the increase of the total amount of blood transfusions. Conclusion there is a significant correlation between the age and the type of operation and the average amount of blood transfusion. We should strictly grasp the indication of blood transfusion and choose safe and economical ways of using blood.
