家庭医生管理模式对2型糖尿病患者健康管理的效果评价

【摘要】：目的探讨在社区应用家庭医生管理模式对糖尿病患者进行健康管理的效果。方法以上海市长宁区天山路街道社区卫生服务中心运用家庭医生分级管理的185例2型糖尿病患者作为研究对象,运用家庭医生管理模式对所有入选的患者进行为期1年的糖尿病分级健康管理。对比家庭医生分级管理前和管理后患者的健康相关行为的改变,以及包括体质指数、血压、空腹血糖、餐后2 h血糖、糖化血红蛋白(Hb A1C)、血脂等在内的健康指标的变化。结果管理1年后患者的空腹血糖由干预前的(8.65±2.31)mmol/L降低至干预后的(6.66±1.49)mmol/L,餐后2 h血糖由干预前的(10.23±3.42)mmol/L降低至干预后的(8.72±2.26)mmol/L,Hb A1C由干预前的(7.42±1.35)%降低至干预后的(6.64±0.85)%,差异均有统计学意义(P0.05)。结论在社区通过家庭医生管理模式对糖尿病患者进行健康管理,能显著地将糖尿病患者血糖控制在正常范围内,有利于预防糖尿病并发症的发生,提高患者的生活质量。
[Abstract]:Objective to explore the effect of applying the family doctor management model to the health management of diabetic patients in the community. Methods 185 patients with type 2 of diabetes mellitus were classified by family doctors in the community health service center of Tianshan road street in Changning district, shanghai. Diabetic patients as subjects of study, The family doctor management model was used to manage all patients with diabetes mellitus for one year. The changes of health-related behaviors, including body mass index (BMI), blood pressure (BP), were compared before and after the management. Fasting blood glucose, 2 hours postprandial blood glucose, Results the fasting blood glucose decreased from 8.65 卤2.31 mmol / L before intervention to 6.66 卤1.49 mmol / L after intervention, and from 10.23 卤3.42 mmol / L before intervention to 8.72 卤2.26 mmol / L after intervention. A1C decreased from 7.42 卤1.35% before intervention to 6.64 卤0.85% after intervention, and the difference was statistically significant (P 0.05). Conclusion Health management of diabetic patients is carried out in the community through family doctor management model. It can significantly control the blood sugar of diabetic patients within the normal range, which is beneficial to prevent the occurrence of diabetic complications and improve the quality of life of the patients.
