不同b值组合时VX2肿瘤ADC值差异的评估及与肌肉的比较

【摘要】:目的评价不同b值条件下所得VX2肿瘤和肌肉组织的表观弥散系数(ADC)值的差异,并比较肿瘤和肌肉组织间各种ADC值的差别,以探讨可鉴别两者的有效ADC值计算方法。方法在15只新西兰兔大腿接种VX2肿瘤,3周后对其行动态对比剂增强磁共振成像(DCE-MRI)和多b值弥散加权成像(DWI);由b=0、50、100 s/mm~2的DWI生成ADC_(low);由b=400、600、800 s/mm~2的DWI生成ADC_(high)。根据增强早期DCE-MRI在ADC_(low)和ADC_(high)图选取感兴趣区测量VX2肿瘤组织和肌肉组织的ADC_(low)和ADC_(high)值。比较VX2肿瘤组织及肌肉的ADC_(low)与ADC_(high)的差别;同时分析VX2肿瘤组织与肌肉组织之间ADC_(low)及ADC_(high)的差异,并比较其ROC曲线下面积。结果 VX2肿瘤组织的ADC_(low)及ADC_(high)分别为(1.378±0.203)×10~(-3)mm~2/s、(1.089±0.098)×10~(-3)mm~2/s,差异有统计学意义(P0.001);肌肉的ADC_(low)及ADC_(high)分别为(1.684±0.059)×10~(-3)mm~2/s、(1.538±0.053)×10~(-3)mm~2/s,差异有统计学意义(P0.001)。VX2肿瘤组织与肌肉之间的ADC_(low)及ADC_(high)的差异也均有统计学意义(P0.001);ADC_(high)与ADC_(low)的ROC曲线下面积分别为1.000和0.903,差异有统计学意义(P=0.023)。结论利用低b值DWI所得ADC值受灌注效应的影响大,存活组织的ADC_(low)高于ADC_(high);对于细胞排列密集的软组织肿瘤,ADC_(high)区别肿瘤和肌肉两种组织的效能较高。
[Abstract]:Objective to evaluate the difference of apparent diffusion coefficient (ADC) of VX2 tumor and muscle tissue under different b values, and to compare the differences of ADC values between tumor and muscle tissue. To investigate the effective ADC value calculation method for differentiating the two. Methods dynamic contrast enhanced magnetic resonance imaging (DCE-MRI) and multi-b diffusion-weighted imaging (DWII) were performed in 15 New Zealand rabbits with VX2 tumor inoculated in their thighs for 3 weeks. The DWI of B400 / 600 / 800 s / mm / mm / 2 is used to produce high ADCS. According to the enhanced early DCE-MRI (ADCS) and ADCS / DCE-MRI (high) charts, the values of ADCS and ADCS in the area of interest are selected. The difference between the VX2 tumor tissue and the muscle is compared with that of the ADCS / ADCS. At the same time, the difference between VX2 tumor tissue and muscle tissue was analyzed.
