小腿肌肉收缩活动的量化方法探索研究

【摘要】:小腿肌肉在走、跑、跳等基本肢体活动中起驱动和控制作用。胫前肌和腓肠肌是小腿肌肉中体积较大的表浅肌,分别负责踝背屈和踝跖屈。然而,目前对二者的状态评估多是从功能角度出发以功能量表的方式完成,一般忽视了结构信息。此外,小腿肌肉的研究普遍存在研究对象年龄跨度大,受试者数量少的问题。与此同时,偏瘫是中枢神经损伤疾病常见的后遗症,典型症状为足下垂,多由前肌运动功能受损所致。临床上常采用电针治疗足下垂,然而治疗过程中,肌肉的收缩活动缺乏量化监控,治疗的刺激持续时间和强度等参数选择严重依赖临床医生的经验和患者的反馈,易造成治疗效果差。鉴此,本文先基于小腿肌肉中的胫前肌和腓肠肌,对健康青年人在性别、肢体左右侧和经常锻炼与否进行了统计差异分析,发现胫前肌有较突出的敏感性,之后开展了对偏瘫患者胫前肌在电针治疗下的收缩活动量化方法进行了相关探索。具体地,本文采用超声影像技术采集了健康青年受试者在不同动作时胫前肌或腓肠肌的超声影像图,并从中提取了肌肉多种参数,发现相比外侧腓肠肌,胫前肌对性别、左右侧、经常运动与否的变化较敏感;对电针治疗下胫前肌量化研究,本文采集了超声图像、肌动图数据和惯性数据,从不同角度采用10种指标探索胫前肌的收缩活动,实验发现,其中6种量化指标,更加适合用于量化分析电针治疗时肌肉的收缩活动,它们反映了随着电针治疗,胫前肌收缩活动的幅度有下降的变化,胫前肌收缩活动的频率有增加的变化。该变化可望为临床上电针治疗的刺激时间的设定提供初步的、量化的信息和依据。综上所述,本文对小腿肌肉收缩活动的量化方法探索研究主要分为两方面:一方面分析了健康青年受试者的胫前肌和外侧腓肠肌因性别、左右侧、经常锻炼与否所存在的差异;另一方面从多种角度探索了电针治疗时,胫前肌的收缩活动情况。希望本文的工作可帮助从事体育科学、康复工程等领域的工作人员对胫前肌和腓肠肌的了解。
[Abstract]:The calf muscles drive and control basic limb movements such as walking, running and jumping. The tibial anterior muscle and the gastrocnemius muscle are the larger superficial muscles in the calf muscles, which are responsible for ankle dorsiflexion and ankle plantar flexion, respectively. At present, most of the state assessment of the two is done by functional scale, and the structural information is generally ignored. In addition, there is a large age span in the study of leg muscles. At the same time, hemiplegia is a common sequelae of central nervous system injury disease. The typical symptom is foot droop, mostly caused by the impairment of anterior muscle motor function. In clinic, electroacupuncture is often used to treat foot droop, however, in the course of treatment, The lack of quantitative monitoring of muscle contractile activity, the treatment of stimulation duration and intensity of the choice of parameters rely heavily on clinicians experience and patient feedback, prone to poor therapeutic effect. Based on the tibial anterior muscle and gastrocnemius muscle in the calf muscles, the statistical differences of gender, left and right limbs and regular exercise in healthy young people were analyzed, and it was found that the tibial anterior muscles had prominent sensitivity. After that, the quantitative method of contraction activity of the anterior tibial muscle of hemiplegic patients under electroacupuncture treatment was explored. Specifically, In this paper, ultrasonic images of tibialis anterior muscle or gastrocnemius muscle in different movements of healthy young subjects were collected, and a variety of muscle parameters were extracted. It was found that compared with lateral gastrocnemius muscle, tibial anterior muscle was gender opposite, right and left. For the quantitative study of electroacupuncture treatment of lower tibial anterior muscle, the ultrasonic images, actin data and inertia data were collected, and 10 indexes were used to explore the contractile activity of the anterior tibial muscle from different angles. Six of them are more suitable for quantitative analysis of muscle contractility during electroacupuncture treatment, which reflect a decrease in the amplitude of the contraction of the anterior tibial muscle with electroacupuncture treatment. The frequency of tibial anterior muscle contraction has increased. This change is expected to provide preliminary, quantitative information and basis for the setting of stimulation time in clinical electroacupuncture therapy. In this paper, the quantitative research on the contractile activity of calf muscle is mainly divided into two aspects: on the one hand, the differences between the tibialis anterior muscle and the lateral gastrocnemius muscle due to sex, left and right side, regular exercise or not were analyzed. On the other hand, the contractile activity of tibialis anterior muscle during electroacupuncture treatment was explored from various angles. It is hoped that the work of this paper can help the workers in sports science, rehabilitation engineering and other fields to understand the tibial anterior muscle and gastrocnemius muscle.
