实时三维超声与斑点追踪技术评价房颤患者射频消融术前后左房结构及功能重构的临床价值

【摘要】:目的探讨实时三维超声心动图(RT-3DE)与斑点追踪技术(STI)评价房颤患者射频消融术前后左房结构及功能重构的临床价值。方法利用RT-3DE分别获得33例阵发性房颤患者消融术前、术后3d、术后6个月及25例健康志愿者的左房最大容积、最小容积、射血分数(LAVmax、LAVmin、LAEF),经STI测得各组左房壁左室收缩期峰值应变率、左室舒张早期峰值应变率、左房收缩期峰值应变率(SRs、SRe、SRa)。结果 (1)与对照组相比,术前LAVmax、LAVmin增大,LAEF、SRs、SRe、SRa减小。(2)术后3d与术前相比,LAVmax、LAVmin、LAEF差异无统计学意义,SRs、SRe、SRa减小。(3)术后6个月与术后3d相比,LAVmax、LAVmin减小,LAEF及SRe、SRa、SRs增大。(4)术后6个月与对照组比较,LAVmax、LAVmin、LAEF、左房前壁、房间隔、下壁SRe、SRa、SRs差异无统计学意义,左房侧壁、后壁SRe、SRa、SRs减小。结论射频消融术后短时间内,处于左房功能顿抑期;术后6个月,左房结构和功能逐渐恢复。RT-3DE及STI对评价消融术前后左房重构有重要价值。
[Abstract]:Objective to evaluate the clinical value of real-time three-dimensional echocardiography (RT-3DE) and speckle tracing technique (STI) in evaluating left atrial structural and functional remodeling in patients with atrial fibrillation before and after radiofrequency ablation. Methods 33 patients with paroxysmal atrial fibrillation were obtained by RT-3DE before ablation. Left atrial maximum volume, minimum volume and ejection fraction of left atrium were measured 3 days after operation, 6 months after operation and 25 healthy volunteers. Left ventricular systolic peak strain rate and early left ventricular diastolic peak strain rate were measured by STI. The peak strain rate of left atrial systolic phase was compared with that of the control group. There was no significant difference between the preoperative LAVmaxand LAVmax / SReVmin and the preoperative LAVmax-LAVmin / LAVmin-LAEF (P < 0.05) 6 months after operation, compared with the postoperative 3 days after operation, the LAVmaxLAVmin decreased and the SReSRA SRS increased. There was no significant difference in SRs between SRS and SRS of the inferior wall, but the SRs of SRS in left atrial wall and posterior wall were decreased. Conclusion the SRs of SRS in the left atrium after radiofrequency ablation for a short period of time are in the period of left atrial functional depression, 6 months after operation, and 6 months after radiofrequency ablation. The gradual restoration of left atrial structure and function. RT-3DE and STI are of great value in evaluating left atrial remodeling before and after ablation.
