DCE-MRI定量参数对子宫肌瘤HIFU治疗效果的即时评估价值

【摘要】:目的:通过研究子宫肌瘤动态增强磁共振成像(Dynamic Contrast Enhanced MRI,DCE-MRI)定量参数与治疗效果的相关性,探讨定量参数对子宫肌瘤高强度聚焦超声(High Intensity Focused Ultrasound,HIFU)疗效即时评价的应用价值。材料与方法:根据纳入和排除标准收集子宫肌瘤HIFU治疗的13个病例,应用Philips Achieva 3.0T磁共振,在治疗前1天内、治疗后1天内行MRI平扫及DCE-MRI检查。应用Omni-Kinetics(GE Healthcare)软件依据双室Tofts模型得到各定量参数值。根据治疗后DCE-MRI的影像表现分为完全消融组和不完全消融组。先采用Mann-Whitney U检验,然后(1)采用配对样本t检验比较各参数治疗前后的差异;(2)应用方差分析比较两组间各参数的差异;(3)应用Spearman秩相关分析研究定量参数与不同分组间的关联。结果:(1)HIFU治疗后,Ktrans、Kep、Ve均明显减低,差异具有统计学意义(p0.05),Vp变化不明显。(2)除Vp_(治疗前-治疗后)外,治疗前所有定量参数与治疗后的定量参数差值(即治疗前-治疗后)与分组无相关性(P0.05)。(3)两组中Ktrans_(治疗后)、Ve_(治疗后)及Ktrans_(变化率)、Ve_(变化率)与分组情况呈相关性,其中Ktrans_(治疗后)、Ve_(治疗后)与分组呈正相关,相关系数分别为r=0.713和0.757;Ktrans_(变化率)、Ve_(变化率)与分组呈负相关,且相关性更显著,相关系数均为r=-0.802。结论:子宫肌瘤HIFU术后的即时疗效评价可以应用DCE-MRI定量参数,定量参数值对临床预后的指导具有参考价值。
[Abstract]:Objective: through the study of enhanced magnetic resonance imaging of uterine leiomyoma (Dynamic Contrast Enhanced dynamic MRI, DCE-MRI) correlation between quantitative parameters and treatment effect, explore the quantitative parameters of Hysteromyoma with high intensity focused ultrasound (High Intensity, Focused Ultrasound, HIFU) application effect of immediate evaluation. Materials and methods: according to inclusion and exclusion criteria 13 cases collection of HIFU in the treatment of uterine fibroids, the application of Philips Achieva 3.0T MRI, in 1 days before treatment, after treatment 1 days after the MRI scan and DCE-MRI examination. The application of Omni-Kinetics (GE Healthcare) software based on double chamber Tofts model get the quantitative parameter values. According to DCE-MRI findings after treatment divided into complete ablation group and no complete ablation group.
