ADC值对早期宫颈癌的诊断价值及与病理对照研究

【摘要】：目的:探讨不同病理类型早期宫颈癌ADC值的差异及其临床意义。方法:选取131例经病理及临床证实为早期宫颈癌的患者,其中鳞癌100例、腺癌17例、腺鳞癌14例。100例鳞癌中低分化4例、中分化87例、高分化7例。所有患者治疗前行3.0T MR常规扫描及DWI检查。测量病灶的ADC值,比较不同病理分型及不同分化程度早期宫颈癌的ADC值的差异。结果:早期宫颈鳞癌与腺癌的ADC均值、最小值之间的差异有统计学意义(P0.01),早期宫颈腺癌与腺鳞癌的ADC均值、最小值之间的差异有统计学意义(P0.01)。分化程度不同的早期宫颈鳞癌之间ADC均值的差异无统计学意义(P0.05)。结论:ADC值在术前评估早期宫颈癌的病理类型方面有一定价值,但对其分化程度的术前评估价值有限。
[Abstract]:Objective: to explore the difference of ADC value and its clinical significance in different pathological types of early cervical cancer. Methods: one hundred patients with squamous cell carcinoma and 17 with adenocarcinoma were selected. There were 4 cases of low differentiation, 87 cases of moderate differentiation and 7 cases of highly differentiated adenosquamous carcinoma. Before treatment, 3. 0T Mr routine scan and DWI examination were performed in all patients. The ADC value of the lesions was measured. Results: the mean value of ADC and the minimum value of ADC in early cervical squamous cell carcinoma and adenocarcinoma were statistically significant (P 0.01), and the ADC mean value of early cervical adenocarcinoma and adenosquamous carcinoma was higher than that of early cervical carcinoma and adenosquamous carcinoma. The difference between the minimum values was statistically significant (P 0.01). There was no significant difference in the mean value of ADC between early cervical squamous cell carcinomas with different differentiation. Conclusion the ratio of ADC value is valuable in the preoperative evaluation of the pathological types of early cervical carcinoma. However, the preoperative evaluation of the degree of differentiation was limited.
