急性胰腺炎患者健康教育效果评价指标体系的构建

【摘要】:目的初步构建一套科学、系统、规范的急性胰腺炎患者健康教育效果评价指标体系,为临床护理人员实施系统、规范的健康教育及全面评价健康教育效果和质量提供参考依据和有效指导。方法在文献回顾、半结构式专家访谈和理论分析的基础上初步拟定急性胰腺炎患者健康教育效果评价指标体系框架。通过德尔菲(Delphi)法对来自护理教育、护理管理和临床护理三个领域的26名专家进行正式的咨询,并应用百分权重法和乘积法对各级指标的权重进行赋值,使各级指标得以量化。最后应用EXCEL2011和SPSS22.0统计分析软件对数据进行收集、整理和分析。结果1.本研究经过两轮德尔菲咨询最终形成急性胰腺炎健康教育效果评价指标体系,其中包括健康知识、健康信念、健康行为、健康教育满意度4个一级指标和31个二级指标。2.两轮问卷回收率均为100%,表明专家积极性较高;两轮德尔菲咨询后,专家权威系数分别为0.84,表明专家具有较高的专业权威性;两轮专家咨询后,一、二级指标的协调系数分别为0.203、0.327,表明专家组对各级指标重要性意见基本协调一致,卡方检验P0.01,差异有统计学意义。3.通过百分权重法和乘积法对各级指标进行权重赋值,健康知识、健康信念、健康行为4个一级指标的权重分别为0.278、0.253、0.309、0.160。4.指标内容分析:第一轮专家咨询根据研究设计的指标取舍标准及专家意见建议,删除“疾病的名称”、“休息时的体位”、“患者对家属的依赖”、“患者对医护人员的依赖”、“翻身的重要性”、“灌肠与导泻的目的”6项二级指标条目,增加“急性胰腺炎的病情认知”、“不同时期活动要求及注意事项(急性期卧床休息目的;缓解期适量活动的意义;术后活动要求)”2项二级指标条目,并将“饮食的要求”改为“不同阶段的饮食要求(急性发作期禁食禁饮的目的;缓解期低脂清淡饮食的重要性;手术患者术前术后饮食要求)”,将“患者对家属的依赖”和“患者对医护人员的依赖”2个二级条目合并为“有寻求医护人员和家属帮助的意愿”。第二轮专家咨询专家意见基本达成一致,最终确立了包含4个一级指标,31个二级指标在内的急性胰腺炎健康教育评价指标体系。结论本研究在文献回顾、理论分析、半结构访谈和德尔菲咨询的基础上,最终形成包括4个一级指标和31个二级指标在内的急性胰腺炎健康教育效果评价指标体系,确定各级指标的权重值,对各级指标进行量化。研究过程中,专家代表性好、积极性高、专家意见基本协调一致,整个研究过程严谨,研究结果科学可靠,可以为临床护士实施健康教育并评价其实施效果和质量提供量化依据,从而提高护士积极性,提高患者自我管理能力及健康教育质量和效果。
[Abstract]:Objective to establish a scientific, systematic and standardized index system for evaluating the effect of health education in patients with acute pancreatitis. Standard health education and comprehensive evaluation of the effectiveness and quality of health education to provide reference and effective guidance. On the basis of semi-structured expert interview and theoretical analysis, the framework of evaluation index system of health education effect in patients with acute pancreatitis was preliminarily drawn up. Twenty-six experts in the three fields of nursing management and clinical nursing conducted formal consultation, and used the percentage weight method and the product method to assign the weights of the indicators at all levels. Finally, the data were collected, collated and analyzed by EXCEL2011 and SPSS22.0 statistical analysis software. Results 1. After two rounds of Delphi consultation, the evaluation index system of health education effect of acute pancreatitis was formed. These include health knowledge, health belief, healthy behavior, satisfaction with health education, and 31 secondary indicators. The recovery rate of the two rounds of questionnaires is 100, indicating that experts are highly motivated, and after two rounds of Delphi consultation, The expert authority coefficient is 0.84, which indicates that the expert has higher professional authority. After two rounds of expert consultation, the coordination coefficient of the first and second level indicators is 0.203 / 0.327, respectively, indicating that the expert group's views on the importance of indicators at all levels are basically coordinated and consistent. Chi square test P0.01, the difference is statistically significant. 3. Weight assignment, health knowledge, health belief through percentage weight method and product method. The weight of the four first-level indexes of health behavior is 0.2780.253U 0.309U 0.160.4.The index content analysis: the first round of expert consultation according to the criteria and recommendations of the criteria designed by the study, Delete "disease name", "rest posture", "patient dependence on family members", "patient dependence on health care workers", "importance of turning over", "Enema and purgation purpose", Two secondary index items, "Cognition of the condition of Acute pancreatitis", "activity requirements and precautions in different periods (purpose of bed rest in acute phase; significance of moderate activity in remission period; requirements for postoperative activity)" were added. "Dietary requirements" should be changed to "Diet requirements at different stages (the purpose of fasting and drinking during acute attack, the importance of low-fat and light diet in remission period," and "Dietary requirements". "preoperative and postoperative dietary requirements of patients", combining "patients' dependence on family members" and "patients' dependence on health care personnel" into "willingness to seek assistance from health care personnel and family members". The agreement was basically reached. Finally, the evaluation index system of acute pancreatitis health education including 4 primary indexes and 31 secondary indexes was established. Conclusion this study is based on literature review, theoretical analysis, semi-structural interviews and Delphi consultation. Finally, an index system for evaluating the effect of health education in acute pancreatitis including 4 primary indexes and 31 secondary indexes was formed, and the weights of the indicators at all levels were determined, and the indexes at all levels were quantified. In the course of the research, the experts were well represented. The research results are scientific and reliable, which can provide quantitative basis for clinical nurses to carry out health education and evaluate the effect and quality of their implementation, so as to improve the enthusiasm of nurses.
