fMRI技术在探索原发性三叉神经痛患者静息态下脑功能变化的研究

【摘要】:目的利用功能性核磁共振成像(f MRI)技术评估三叉神经痛患者处于静息状态下大脑自发性功能活动变化。方法选择原发性三叉神经痛(ITN)患者组26例,健康对照组22例,受试者静息态下进行功能性核磁共振扫描。利用DPARSF软件对图像进行处理,得到两组人员的全脑平均低频振幅率(mf ALFF)值。使用SPM12和SPSS 17.0软件分析数据。结果两组受试人员在年龄、受教育程度和男女比例上差异无统计学意义;ITN患者组部分脑区mf ALFF值明显高于健康对照组,分别是:左侧枕中回(BA18)、枕下回(BA19)、三角部额下回(BA47)、梭状回(BA37)、小脑脚1区、距状裂周围皮层、中央旁回,右侧背外侧额上回(BA8),差异有统计学意义(P0.01)。结论 ITN患者多个相关脑区的mf ALFF值增高,提示患者在静息状态下,部分脑区的活动改变可能与疼痛的调节或感觉的整合有关。
[Abstract]:Objective to evaluate the changes of spontaneous brain function in patients with trigeminal neuralgia by functional magnetic resonance imaging (fMRI) technique. Methods 26 patients with primary trigeminal neuralgia and 22 healthy controls were selected. The subjects were scanned by functional nuclear magnetic resonance (fMRI) under resting state. The mean low frequency amplitude rate of the whole brain was obtained by using DPARSF software. The data were analyzed by SPM12 and SPSS 17.0 software. There was no significant difference in education level and ratio between men and women. The values of MF ALFF in some brain regions in ITN group were significantly higher than those in healthy control group. They were: left middle occipital gyrus BA18, inferior occipital gyrus 19, triangle inferior frontal gyrus BA47, fusiform gyrus BA37, cerebellar foot 1. There was significant difference in the paracentral cortex, paracentral gyrus and right dorsolateral superior frontal gyrus (P 0.01). Conclusion the increase of MF ALFF in several related brain regions in ITN patients suggests that the patients are in resting state. Changes in activity in parts of the brain may be related to pain regulation or sensory integration.
