低频振幅fMRI联合基于体素的形态学分析在精神分裂症中的应用

【摘要】:目的探讨静息态低频振幅(ALFF)功能性磁共振及基于体素的形态学分析(VBM)在精神分裂症脑功能与结构变化中的应用价值。方法符合入选标准的32例精神分裂症患者(患者组)和32例健康志愿者(对照组)进行静息态下脑功能磁共振成像(Rs-f MRI)扫描及矢状位薄层T1加权数据采集;采用Rest1.8软件对患者组和对照组标准化后ALFF的脑图进行分析对比ALFF值差异;再通过对存在差异的ALFF异常脑区进行患者组和对照组的VBM灰质密度数据进行统计分析灰质密度图像差异;最后结合ALFF及VBM结果,探讨精神分裂症患者脑功能及脑结构的改变。结果与对照组相比,患者组ALFF值显著增高(P0.05)的脑区是右侧壳核、左侧眶额回延伸至左侧壳核和左侧背外侧额上回;ALFF值显著降低(P0.05)的脑区是右侧颞下回、左侧扣带回后部、左侧角回。与对照组相比,患者组灰质密度值减低(P0.05)的脑区是左侧角回、右侧楔前叶延伸至左侧扣带回后部、右侧壳核、右侧颞下回、左侧眶额回延伸至左侧壳核、右侧内侧额上回;未发现患者组灰质密度增高脑区。结论精神分裂症患者在静息态下存在较广泛的异常活动脑区,且异常活动脑区的灰质密度普遍减低;该结果与精神分裂症谷氨酸假说中神经元受到广泛损伤,神经递质循环回路失控理论较为一致。
[Abstract]:Objective to investigate the value of functional magnetic resonance imaging (fMRI) and morphologic analysis based on voxel (VBM) in the functional and structural changes of the brain in patients with schizophrenia.Methods 32 schizophrenic patients (patient group) and 32 healthy volunteers (control group) were examined with Rs-f MRI and T1-weighted sagittal data acquisition.Rest1.8 software was used to analyze and compare the ALFF values of ALFF between the patient group and the control group.Then the difference of gray matter density image of VBM in patients and control group was statistically analyzed by analyzing the difference of VBM gray matter density data in abnormal ALFF brain area. Finally, the changes of brain function and brain structure in schizophrenia patients were discussed by combining the results of ALFF and VBM.Results compared with the control group, the right putamen nucleus was the brain area in which the ALFF value was significantly increased (P 0.05). The left orbitofrontal gyrus extended to the left putamen nucleus and the left dorsolateral superior frontal gyrus decreased significantly (P 0.05). The right inferior temporal gyrus and the left posterior cingulate gyrus were the brain regions.Left corner gyrus.Compared with the control group, the left angle gyrus was the brain area in which the gray matter density decreased (P0.05). The right anterior cuneate lobe extended to the posterior part of the left cingulate gyrus, the right putamen nucleus, the right infratemporal gyrus, the left orbital frontal gyrus to the left putamen nucleus and the right medial superior frontal gyrus.No increase in gray matter density was found in the patient group.Conclusion there are a wide range of abnormal active brain areas in schizophrenic patients, and the density of gray matter in abnormal active brain regions is generally decreased, which is associated with the extensive injury of neurons in the schizophrenia glutamate hypothesis.The theory of neurotransmitter circulatory loop runaway is consistent.
