磁共振成像诊断产前先天性畸形的临床效果

【摘要】:目的研究磁共振成像(MRI)技术在产前先天性畸形诊断中的临床效果。方法选取2014年1月-2016年4月该院筛查的产妇13 874例为研究对象,产妇入院后先行常规超声检查,然后采用MRI筛查,比较两种诊断方法筛查效果。结果 13 874例孕妇中384例经手术确诊为胎儿畸形,确诊率为2.77%(384/13 874);26例漏诊,漏诊率为6.77%(26/384)。MRI确诊率为95.8%,显著高于常规超声检查的82.6%,两者比较差异有统计学意义(P0.05);MRI检查漏诊率误诊率为4.2%,显著低于常规超声检查的17.4%,两者比较差异有统计学意义(P0.05);MRI和超声检查畸形类型较多,MRI检查排在前3位的分别为:中枢神经系统、心脏畸形、脸部畸形;常规超声检查排在前3位的分别为:其他、中枢神经系统、心脏畸形,两种方法诊断畸形类型比较差异有统计学意义(P0.05)。结论产前先天性畸形胎儿采用MRI筛查效果理想,但应严格把握其应用适应证,联合超声检查能进一步弥补各自缺陷,提高确诊率。
[Abstract]:Objective to study the clinical effect of magnetic resonance imaging (MRI) technique in prenatal diagnosis of congenital malformation. Methods from January 2014 to April 2016, 13 874 cases of puerpera who were screened in our hospital were selected. After admission, routine ultrasound examination was performed, and then MRI screening was used to compare the results of the two diagnostic methods. Results of the 13 874 pregnant women, 384 cases were diagnosed as fetal malformation by operation, and the diagnosis rate was 2. 774 / 13 874 misdiagnosis in 26 cases. The missed diagnosis rate of 6.77%(26/384).MRI was 95.8, which was significantly higher than that of conventional ultrasound (82.6%). There was a significant difference between the two groups. The misdiagnosis rate of P0.05 was significantly lower than that of conventional ultrasound, and the misdiagnosis rate was significantly lower than that of conventional ultrasound. The difference was statistically significant. The most common types of deformities in ultrasonography and MRI were as follows: central nervous system (CNS). There were significant differences between the two methods in the diagnosis of cardiac malformations and facial deformities, and in the first three places by conventional ultrasound examination: others, central nervous system and heart malformations, and there was significant difference between the two methods in the diagnosis of deformities (P 0.05). Conclusion MRI screening is ideal for prenatal congenital malformation fetus, but the indication should be strictly grasped. Combined ultrasound examination can make up for their defects and improve the diagnosis rate.
