造影时间-强度曲线在三阴性乳腺癌鉴别诊断中应用的初步探讨

【摘要】:目的:初步探讨超声造影时间-强度曲线(TIC)在三阴性乳腺癌(TNBC)鉴别诊断中的应用价值。方法:以病理及免疫组化结果为金标准,分析126例患者共126个病灶及其周围组织的超声造影TIC。结果:TNBC病灶增强强度及曲线下面积均大于非TNBC(P0.001),其ROC曲线下面积分别为0.857,0.807,初步界定当病灶增强强度11.99时,其为TNBC可能性大(灵敏度88%,特异度80%),当病灶曲线下面积684.31时,其为TNBC可能性大(灵敏度60%,特异度95%);TNBC组周围组织与非TNBC组周围组织对比,其基础强度较低,增强强度较大(P0.05);与周围组织比较,TNBC组病灶的峰值强度、增强强度、上升支斜率、峰值减半差值时间、曲线下面积大于周围组织,两两比较有统计学差异(P0.05),但不同于非TNBC,其始增时间、达峰时间、基础强度与周围组织比较无明显差异。结论:应用TIC定量分析可在一定程度上帮助鉴别TNBC与非TNBC。
[Abstract]:Objective: to evaluate the value of time intensity curve (TIC) in differential diagnosis of triple negative breast cancer. Methods: TICs of 126 lesions and surrounding tissues in 126 patients were analyzed by using pathological and immunohistochemical results as gold standard. Results both the enhancement intensity and the area under the curve were larger than those of the non-TNBCU (P 0.001), and the areas under the ROC curve were 0.857 / 0.807, respectively. It was preliminarily defined that when the enhancement intensity of the lesion was 11.99, it was more likely to be TNBC (sensitivity 88%, specificity 80%), and when the area under the focus curve was 684.31, The results showed that there was a great possibility of TNBC (sensitivity 60%, specificity 95%). Compared with non-TNBC group, the basic strength was lower and the enhancement intensity was larger (P 0.05), and the peak intensity, enhancement intensity and ascending branch slope were higher than those in the surrounding tissue. The area under the curve was larger than that of the surrounding tissue, and the difference was statistically significant (P 0.05), but different from that of non-TNBC, there was no significant difference in initial increase time, peak reaching time and base strength between the two groups. Conclusion: quantitative analysis of TIC can help to distinguish TNBC from non TNBC to some extent.
