MR扩散加权成像定量化分析在预测小肝癌分化程度的价值

【摘要】:目的探讨磁共振扩散加权成像(DWI)定量化分析在不同分化程度小肝癌(sHCC)的诊断价值。方法将经病理证实的36例sHCC纳入研究,所有患者术前均行常规MRI和DWI(b=0,600 s/mm~2)检查。测量瘤灶的DWI-ADC值,信号强度(SI)值及信号相对比值(RCR),并探讨其与sHCC不同分化程度的关系。结果 36例sHCC患者,包括高分化组(n=20)、中分化组(n=10)和低分化组(n=6)。高、中和低分化组的sHCC病灶的平均ADC值分别为(1.46±0.68)×10~(-3)mm~2/s、(1.32±0.56)×10~(-3)mm~2/s和(1.13±0.63)×10~(-3)mm~2/s,各组间差异具有统计学意义。高、中和低分化组的sHCC病灶的平均SI值分别为87.73±4.82、72.73±8.04和89.13±5.98;而平均RCR值分别为1.37±0.13、1.57±0.13和1.71±0.10。SI值高、低分化组间差异无统计学意义,而RCR值在各组间差异均有统计学意义。ROC曲线显示ADC值较SI值更有助于区分高分化与中低分化sHCC,其曲线下面积为0.964;当ADC值≥1.39×10~(-3)mm~2/s时,判断高分化HCC的敏感性为85%,特异性为93.7%。结论基于DWI技术的ADC值能有效判断不同分化程度s HCC,而SI值提供相对有限的价值。
[Abstract]:Objective to evaluate the diagnostic value of diffusion weighted Mr imaging (DWI) in the diagnosis of small hepatocellular carcinoma (HCC) with different differentiation. Methods 36 cases of sHCC confirmed by pathology were included in the study. All patients were examined by routine MRI and DWI(b=0600 s / mmm-2 before operation. The DWI-ADC value, signal intensity and signal relative ratio of tumor focus were measured, and the relationship between them and the differentiation degree of sHCC was discussed. Results 36 patients with sHCC, including well-differentiated group, moderately differentiated group, and poorly differentiated group, were included. The mean ADC values of sHCC lesions in the high, middle and low differentiation groups were 1.46 卤0.68) 脳 10 ~ (-3) / 2 ~ (-1) 10~(-3)mm~2/s and 1.13 卤0.63) 脳 10 ~ (10) ~ (-3) mm ~ (-1) / s, respectively. The difference was statistically significant among the three groups. There was no significant difference between the low differentiation group and the low differentiation group. The difference of RCR value among all groups was statistically significant.
