基于卓越绩效评价准则的ICU护理质量评价指标体系的构建

【摘要】:研究目的以卓越绩效模式作为概念框架,构建一套切合实际、具有较高信度和效度的重症监护病房护理质量评价指标体系,增强对ICU护理品质管理方面的标准化水平,推动重症监护室护理服务品质的不断增强。研究方法经过查找中国知网、万方数据库、pubmed等医学数据库后,收集ICU护理质量相关指标,借鉴卓越绩效管理模式在其他行业中运行成功的方式,与《卓越绩效模式评价准则运行说明》相对照,经过专家小组的研究与探讨,首先制定有关ICU护理质量评价指标框架。运用德尔菲专家咨询法开展德尔菲专家咨询。经过两次德尔菲法,最后确定ICU护理质量指标体系。在进行完第二次的专家咨询后,运用变异系数法计算出每个指标权重。研究结果运用总体分析概括,探讨有关我国以及其他国家医院内使用绩效管理和评价、护理质量评价指标体系建立的有关方式以及理论知识,将国际著名的卓越绩效模式评判管理标准中的重要价值观与理论结构进行参考,在经过专家多次分析讨论与咨询后,得出以下研究成果:1.通过对国内医院ICU护理质量评价指标体系的研究进展和现状的分析,针对医院ICU护理管理出现的诸多问题,将系统理论思想引入ICU护理质量管理中,提出借鉴国外优秀医院以及企业应用卓越绩效模式后的经验,学习将卓越绩效模式导入护理质量评价中。2.创建ICU护理质量指标体系理论框架:组织的领导、组织的战略与患者需求、资源配置、过程管理与结果。3.遵循卓越绩效框架模式中的原则,分析评价标准,初步形成了 ICU护理质量评价指标体系:一级指标6个、二级指标30个、三级指标91个,并确定了各指标的权重系数。研究结论1.把卓越绩效这一模式运用到国内ICU护理质量评价中,丰富了 ICU护理质量评价的内容,并使其内涵大大提升;2.强调以患者为中心在ICU护理质量评价指标体系中的核心定位,对ICU的功能定位和管理发展起到有益的导向作用;3.在成熟、先进的理论框架基础上建立ICU护理质量评价指标体系,使指标体系的内容更加丰富、覆盖面更广,弥补了以往研究的不足;4.指标的筛选和确定权重方法科学、合理,专家积极系数、权威系数可靠程度高。
[Abstract]:Objective to construct a set of practical, reliable and effective evaluation index system of nursing quality in intensive care unit (ICU) with excellent performance model as the conceptual framework, and to enhance the standardization level of nursing quality management in ICU. To promote the quality of nursing service in ICU. Methods after looking up the medical databases such as China Zhiwang, Wanfang database and pubmed, the authors collected the relevant indexes of ICU nursing quality, and used the excellent performance management model for reference to run successfully in other industries. Compared with the operating instructions of the excellent performance model evaluation criterion, the framework of the ICU nursing quality evaluation index was established through the research and discussion of the expert group. Use Delphi expert consultation method to carry out Delphi expert consultation. After two Delphi methods, the ICU nursing quality index system was determined. After the second expert consultation, the coefficient of variation method was used to calculate the weight of each index. The results of the study were summarized by general analysis, and the methods and theoretical knowledge of performance management and evaluation in hospitals in China and other countries, and the establishment of evaluation index system of nursing quality were discussed. This paper refers to the important values and theoretical structure in the management standard of the internationally famous outstanding performance model. After many experts' analysis and consultation, the following research results: 1: 1 are obtained. Based on the analysis of the research progress and present situation of the evaluation index system of ICU nursing quality in domestic hospitals, and aiming at the problems in the nursing management of ICU, the systematic theory thought was introduced into the nursing quality management of ICU. The experience of applying excellent performance model to foreign hospitals and enterprises was put forward, and the excellent performance model was introduced into nursing quality evaluation. Establish the theoretical framework of ICU nursing quality index system: organizational leadership, organizational strategy and patient needs, resource allocation, process management and results. 3. According to the principle of excellent performance framework, the evaluation criteria were analyzed and the evaluation index system of ICU nursing quality was preliminarily formed: 6 first-grade indexes, 30 second-level indicators and 91 third-level indicators. The weight coefficients of each index were determined. Conclusion 1. Applying the model of excellent performance to the evaluation of ICU nursing quality in China enriches the content of ICU nursing quality evaluation and greatly promotes the connotation of ICU nursing quality evaluation. It is emphasized that the core orientation of patient centered in the evaluation index system of ICU nursing quality plays a beneficial guiding role in the function orientation and management development of ICU. The evaluation index system of ICU nursing quality was established on the basis of mature and advanced theoretical framework, which made the content of the index system more abundant and the coverage wider, which made up for the deficiency of previous research. The methods of index selection and weight determination are scientific, reasonable, expert positive coefficient and authority coefficient reliability.
