Valsalvas闭气加压MRI在评价咽鼓管功能中的价值研究

【摘要】:目的 评估Valsalvas闭气加压MRI扫描法对咽鼓管功能诊断价值。方法 选取慢性鼻窦炎合并中耳乳突炎者30例(研究组)和健康志愿者20例(对照组)为研究对象。两组均于Valsalvas加压前、后进行咽鼓管MRI扫描。观察Valsalvas加压前后咽鼓管咽口、深部咽鼓管的形态特点,对其形态进行分类,Ⅰ类:潜在间隙型咽鼓管、浅三角形咽口;Ⅱ类:小喇叭状咽口及裂隙状咽鼓管、管状咽鼓管。测量咽口宽度、深度及深部咽鼓管宽度。结果 对照组40个咽口中,Valsalvas加压前有Ⅰ类咽鼓管13个,Ⅱ类咽鼓管27个,Valsalvas加压后有Ⅰ类2个,Ⅱ类咽口38个,与加压前形态差异有统计学意义(χ2=5.02,P0.05)。研究组60个咽鼓管中,闭气前Ⅰ类咽口38个,Ⅱ类22个,闭气后Ⅰ类咽口39个,Ⅱ类21个;Valsalvas加压前后其形态差异无统计学意义(χ2=0.85,P0.05)。Valsalvas加压后研究组咽鼓管咽口平均宽径(3.95±0.37)mm,对照组咽鼓管咽口平均宽径(4.78±0.58)mm,两组间差异有统计学意义(t=1.95,P0.05)。结论 Valsalvas加压MRI可以有效评价咽鼓管功能。
[Abstract]:Objective to evaluate the value of valsalvas gas closure and compression MRI in diagnosis of eustachian tube function. Methods 30 cases of chronic sinusitis complicated with middle ear and mastoiditis (study group) and 20 healthy volunteers (control group) were selected. Both groups underwent MRI scanning of eustachian tube before Valsalvas pressure. The morphological characteristics of the pharyngeal and deep eustachian tubes before and after Valsalvas pressure were observed and classified into three categories: latent interstitial eustachian tubes, shallow triangular pharyngeal orifices, small trumpet pharyngitis and fissured eustachian tubes, and tubular eustachian tubes.
