临床输血红细胞O∶T比值分析

【摘要】：目的统计分析本院血液申请/实际输血(O∶T)比值,提高临床科学合理用血。方法通过血液管理系统按照时间顺序统计本院所有临床科室的血液申请和实际输血的量(悬浮红细胞以单位血U表示),时间自2010年1月-2015年12月共6年期间全院临床科室血液申请和实际输血量,按照科室进行列表,全部科室分为手术科室和非手术科室2部分,计算全院、手术科室、非手术科室以及每个科室6年O∶T均值;同时计算血液申请和实际输血之间的差(O-T,单位U),根据实际合血的成本计算O-T部分产生的成本费用。结果全院6年期间的O∶T为2.043±0.189,每年期间无明显规律;手术科室O∶T比值(3.061±0.362)显著高于全部科室(2.043±0.189)(P0.01)和非手术科室(1.534±0.160)(P0.01)。手术科室中O∶T比值最高的科室为胸外科,其次为神经外科、妇产科和眼科,均显著高于手术科室均值(P0.01),O∶T比值最低的科室为烧伤整形科,显著低于手术科室均值(P0.01)。非手术科室O∶T比值最高的为消化内科,显著高于非手术科室的均值(P0.01)。全院6年期间每年平均O-T差值近9千U,每年其直接耗费的检测费用在40万左右。结论全院O∶T比值接近AABB高限,非手术科室O∶T比值相对合理,手术科室O∶T比值显著升高,手术科室合理用血还需输血科和手术科室之间加强合作进一步提高临床科学合理用血。
[Abstract]:Objective to analyze the ratio of blood application to actual blood transfusion (O: t) in order to improve the rational use of blood in clinical science. Methods according to the time order of blood management system, the blood application and actual blood transfusion volume (expressed as unit blood U) of all clinical departments of our hospital were counted. The time from January 2010 to December 2015 was 6 years. Blood applications and actual blood transfusions in clinical departments, According to the list of departments, all the departments were divided into two parts: the surgical department and the non-operative department. The mean value of O _ (1) T was calculated for 6 years in the whole hospital, the surgical department, the non-operative department and each department. At the same time, the difference between blood application and actual blood transfusion (O-T, unit U),) is calculated according to the actual cost of blood combination. Results the O _ (1) T of the whole hospital was 2.043 卤0.189, and the ratio of O _ (1) to T in surgical department (3.061 卤0.362) was significantly higher than that in all departments (2.043 卤0.189) and non-operative department (1.534 卤0.160) (P0.01). The department with the highest O: t ratio was thoracic surgery, followed by neurosurgery, obstetrics and gynaecology and ophthalmology, which was significantly higher than that in surgery department (P0.01) and the lowest of O: t ratio was in burn department (P0.01), which was significantly lower than that in surgical department (P0.01). The highest of O: t ratio in non-surgical department was in digestive department, which was significantly higher than that in non-operative department (P0.01). The average value of O-T in the whole hospital is about 9,000 UU per year, and the direct cost of detection is about 400000 per year. Conclusion the O: t ratio of the whole hospital is close to the high limit of AABB, the ratio of O: t is relatively reasonable in the non-operative department, and the ratio of O: t in the department of surgery is significantly higher than that in the department of surgery. It is necessary to strengthen the cooperation between the blood transfusion department and the operation department to further improve the rational use of blood in clinical science.
