有关新乡某医院内科医疗部门护理人员胰岛素相关知识的调查

【摘要】:背景:糖尿病是21世纪全球最大的公共卫生紧急事件之一,对于工业化国家和发展中国家来说是一个重大的公共医疗挑战。近来,随着新型胰岛素制剂的引入和对血糖控制的重视,住院治疗胰岛素变得越来越复杂。如何正确使用胰岛素以及避免不正当使用胰岛素所引发的潜在危险对我们来说也是一个挑战。缺乏正确胰岛素的知识将引发本可避免的并发症,加重的患者病情,违反治疗方案,无法达到控制血糖的目的。目标:调查收集新乡某医院的护理人员对胰岛素治疗的相关知识。明确调查对象的个人资料:目前的工作职位,管理糖尿病患者的舒适程度,胰岛素治疗信息的来源。衡量调查对象的胰岛素治疗知识水平。并测试调查对象的胰岛素治疗知识水平是否与他们的目前所担任的职务有一定关系。调查方法:调查对象为新乡某医院内科及其下属部门(重症监护室,心脏科加护病房,急诊科和病房重症监护室)的护士(n=50人),问卷调查采取匿名方式进行,共有25个单项选择题。这些问题涉及以下三个方面:调查对象的专业及职务,胰岛素特征和使用方法以及胰岛素的制备和管理。数据分析结果以平均值和标准值的偏差指示数据频率和百分比表示。在研究分类数据的相关性时,运用了卡方检验的独立性研究方法。结果:根据本实验所采用的评分系统和对调查对象的回答进行分析研究,得出以下结论,66%的调查对象被归类为具有较低的胰岛素知识水平,在22个与胰岛素相关的问题中,他们答案的正确率为27%-54%不等。32%的调查对象具有一般的胰岛素知识水平,正确率为55%-81%不等。没有一个调查对象能够很高的分数,但是有一个调查对象得分极其差。所有病房护士的平均得分(即22个知识问题的平均得分)为11.95,是在以职务为分组标准的对比研究中得分最高的一组。而ER护士(9.8)和ICU护士(9.7)的平均得分大致持平。心脏科加护病房护士平均得分为8.4。在CCU护士中存在2.4分的最低分,最高分出现在病房护士中,为5.5分。ICU护士和ER护士得分分别为3.8和3.4。就胰岛素制剂和管理的问题,各分组内的调查对象水平差别不大。结论:本研究的结论是,调查对象所掌握的有关胰岛素的知识存在相当大的差距,并且水平较低。因此,本研究建议要进行有条理,持续和实时举行会议和研讨会并且要设计和定期更新标准化的医疗方案和治疗方案。
[Abstract]:Background: diabetes is one of the largest public health emergencies in the world in the 21st century. It is a major public health challenge for both industrialized and developing countries. Recently, with the introduction of new insulin preparations and the importance of blood glucose control, insulin hospitalization has become more and more complex. How to use insulin correctly and avoid the potential danger caused by improper use of insulin is also a challenge. Lack of proper knowledge of insulin can lead to avoidable complications, exacerbation of the patient's condition, violation of the treatment plan, and can not achieve the purpose of blood sugar control. Objective: to investigate the knowledge of insulin therapy among nurses in a hospital in Xinxiang. Identify the subjects' personal data: current job positions, management of diabetic comfort, and sources of insulin therapy information. The knowledge level of insulin therapy was measured. The subjects' knowledge of insulin therapy was also tested to determine whether it was related to their current position. Methods: a total of 50 nurses in the Department of Internal Medicine and its subordinate departments (intensive Care Unit, Cardiac intensive Care Unit, Emergency Department and intensive Care Unit) in a hospital in Xinxiang were investigated. The questionnaire was conducted anonymously. There are 25 multiple choice questions. These questions relate to the following three aspects: the subject's profession and position, insulin characteristics and methods of use, and the preparation and management of insulin. The results of the data analysis are expressed as deviations of the mean and standard values indicating the frequency and percentage of the data. The independence method of chi-square test is used to study the correlation of classified data. Results: according to the scoring system used in this study and the responses of the subjects, the following conclusions were drawn: 66% of the subjects were classified as having low level of insulin knowledge, out of 22 questions related to insulin, 66% of the subjects were classified as having a low level of insulin knowledge. The correct rate of their answers ranged from 27% to 54%. 32% of the respondents had general knowledge of insulin, and the correct rate ranged from 55% to 81%. None of the respondents scored well, but one scored badly. The average score of nurses in all wards (i.e. the average score of 22 knowledge questions) was 11.95, which was the highest in a comparative study based on position. The average scores of ER nurses (9.8) and ICU nurses (9.7) were roughly the same. The average score of nurses in cardiac intensive care unit was 8.4. The lowest score of 2.4 was found in CCU nurses, and the highest score was 5.5 in ward nurses. The scores of ICU nurses and ER nurses were 3.8 and 3.4 respectively. As for insulin preparation and management, there was little difference in the level of subjects in each subgroup. Conclusion: there is a wide gap and low level of knowledge about insulin. Therefore, this study recommends structured, continuous and real-time meetings and seminars and the design and regular updating of standardized medical and treatment protocols.
