应用血流向量成像技术对房间隔缺损患者心腔内能量损耗的定量评估

目的:应用血流向量成像技术观察房间隔缺损患者心腔内的血流能量损耗和血流动力学的改变。方法:该研究共收集健康志愿者52例(平均年龄48±11岁),房间隔缺损患者52例(平均年龄37±16岁),其中33例房间隔缺损患者(女性25例;平均年龄39±17岁;年龄跨度9-62岁)接受了经皮房间隔缺损Amplatzer封堵术治疗。应用Aloka Prosound F75型彩色多普勒超声诊断仪采集心尖四腔心二维图像,并通过DAS-RS1工作站进行分析,确定收缩期、舒张早期、心房收缩期三个时期对应的帧。通过描计感兴趣区分别测得右心房、右心室、左心房、左心室在不同时期的血流能量损耗值。结果:房间隔缺损患者右心房、右心室、左心房、左心室的容积和面积均较对照组增大(P<0.01)。在收缩期、舒张早期和心房收缩期,房间隔缺损患者的右心房、右心室、左心房、左心室的能量损耗均较对照组增加(P<0.05)。接受封堵治疗后,患者各心腔内的能量损耗在各时期均较术前减低(P<0.01)。逐步多元回归结果显示在房间隔缺损患者中,收缩期、舒张早期、心房收缩期的能量损耗与右房容积、左房每搏输出量、左室射血分数...
