慢性阻塞性肺疾病急性加重期患者肺部真菌感染对血清炎症因子的影响

【摘要】:目的研究慢性阻塞性肺疾病急性加重期患者肺部真菌感染对患者血清炎症因子和肿瘤坏死因子的影响。方法选取2014年6月-2016年4月慢性阻塞性肺疾病急性加重期合并真菌感染的患者48例为AECOPD感染组,另外50例慢性阻塞性肺疾病且无真菌感染的COPD患者为未感染组,同时取50例健康志愿者为对照组,检测感染组与未感染组患者的肺功能并进行分级,以及感染组患者感染的病原菌菌种。检测各组患者血清炎症细胞因子(IL-1β、IL-2、IL-4、IL-6、IL-8、IL-10)、肿瘤坏死因子(TNF-α)、干扰素(IFN-γ)的表达量,对比三组患者血清炎症细胞因子及不同肺功能分级间血清炎症因子之间的差异。结果与对照组相比,感染组与未感染组患者血清炎症因子(IL-2、IL-6)及肿瘤坏死因子TNF-α含量显著升高,其中感染组IL-2、IL-6及TNF-α含量最高,显著高于未感染组,IFN-γ含量显著降低(P0.05)。此外,未感染组患者不同肺功能分级间血清IL-1β、TNF-α存在显著差异,以Ⅲ级IL-1β、TNF-α含量最高(P0.05);急性感染组患者不同肺功能分级间血清IL-2、IL-4及TNF-α差异有统计学意义,以Ⅲ级IL-2、IL-4含量最高(P0.05)。结论血清IL-2、IL-6、TNF-α含量测量对AECOPD合并真菌感染患者病情的评估及感染预测具有重要临床意义。
[Abstract]:Objective to study the effect of pulmonary fungal infection on serum inflammatory factor and tumor necrosis factor in patients with acute exacerbation of chronic obstructive pulmonary disease. Methods from June 2014 to April 2016, the patients with acute exacerbation of chronic obstructive pulmonary disease were selected. 48 patients with fungal infection were infected with AECOPD. Another 50 COPD patients with chronic obstructive pulmonary disease without fungal infection were selected as control group and 50 healthy volunteers as control group. The pulmonary function of infected and uninfected patients was measured and graded. The levels of serum inflammatory cytokines (IL-1 尾, IL-2, IL-4, IL-6, IL-8, IL-10, TNF- 伪, IFN- 纬) in patients with infection were detected, and the expression levels of IL-10, TNF- 伪 and IFN- 纬 were measured. Results compared with the control group, the levels of serum inflammatory cytokines and tumor necrosis factor TNF- 伪 in the infected and uninfected patients were significantly higher than those in the control group. The levels of IL-6 and TNF- 伪 in the infected group were the highest, which were significantly higher than those in the uninfected group. In addition, there were significant differences in serum IL-1 尾 -TNF- 伪 between different lung function grades in the uninfected group. The level of TNF- 伪 was the highest in grade 鈪,
