气胸肺压缩比的CT测量研究

【摘要】:目的探讨气胸患者CT横断面上气胸最大宽度(A值)和经膈顶胸廓最大前后径(B值)的比值与肺压缩比的相关性。方法搜集本院2015年至2016年内42例气胸患者资料,在CT后处理软件上(ADW 4.5)通过最小密度投影(Min P)分别重组并测出患侧原肺容积V_1和压缩后的肺组织容积V_2,算出气胸空气容积(V_1-V_2),得出肺压缩比[((V_1-V_2)/V_1]%,再测量A值及B值,求出A/B值。通过SPSS 19.0统计学软件先绘制出散点图,求出变量A/B与肺压缩比的相关系数r值,再进一步通过线性回归方程y=a+bx得出常数项a和回归系数b值。结果通过SPSS 19.0统计学软件绘制出的散点图呈线性相关,相关系数r=0.90,模型P0.005,有统计学意义,说明CT横断面上气胸最大宽度和胸廓最大前后径的比值与肺压缩比存在正相关关系。通过线性回归方程y=a+bx得出常数项a=0.024,回归系数b=1.096,进而得出气胸肺压缩比=0.024+1.096×A/B。结论在临床工作中只要测出CT横断面气胸最大宽度及经膈顶的胸廓最大前后径,便可以测算出肺压缩比。使得气胸的肺压缩比计算快捷方便准确。
[Abstract]:Objective to investigate the correlation between the ratio of the maximum width of pneumothorax on CT cross section (A value) and the maximum anterior and posterior diameter of transphrenic top chest (B value) and the lung compression ratio. Methods the data of 42 patients with pneumothorax from 2015 to 2016 in our hospital were collected. ADW 4.5on CT post-processing software, respectively, the original lung volume V _ (1) and compressed lung tissue volume V _ (2) of the affected side were reorganized and measured by minimum density projection (Min P), and the pneumothorax air volume (V _ 1-V _ (2)) was calculated, and the lung compression ratio [V1-V _ (2) / V _ 2 / V _ 1] was obtained, then the A value and B value were measured. Using the SPSS 19.0 statistical software to draw the scatter plot, the correlation coefficient r value between the variable A / B and the lung compression ratio was obtained, and the correlation coefficient r between the variable A / B and the lung compression ratio was obtained. Furthermore, the constant term a and the regression coefficient b were obtained through the linear regression equation yttria BX. Results the scattered plot drawn by SPSS 19.0 statistical software showed linear correlation, the correlation coefficient was 0.90, and the model was P0.005, which had statistical significance. The ratio of maximal width of pneumothorax and the ratio of maximal anterior and posterior diameter of chest on CT cross-section is positively correlated with the ratio of lung compression. By using the linear regression equation yaabx, the constant term a0. 024, the regression coefficient b1. 096, and the pneumothorax lung compression ratio of 0. 024 脳 A / A / B are obtained. Conclusion in clinical work, we only need to measure the maximum width of CT cross section pneumothorax and the maximum anteroposterior diameter of chest through diaphragm roof. The lung compression ratio of pneumothorax can be calculated quickly, conveniently and accurately.
