肺结核患者氧化应激水平和膳食摄入情况分析

【摘要】:目的本文通过调查了解某地区肺结核患者机体氧化应激水平及维生素A、E和β-胡萝卜素营养状况,同时调查肺结核患者日常膳食摄入状况,为指导患者合理膳食,改善肺结核患者营养不良、提高机体抗病能力和治疗效果提供科学依据。 方法随机抽取临沂市某县肺结核防治所2010年就诊的70例肺结核患者作为病例组,匹配选取该地同期正常健康人群70例作为对照组。两组人群均进行相关营养指标检测,包括基本营养状况,如体质指数(BMI)、血红蛋白(Hb);血脂水平分析,如甘油三酯(TG)、总胆固醇(TCHO)、高密度脂蛋白(HDL);抗氧化酶水平分析,包括超氧化物歧化酶(SOD)和过氧化氢酶(CAT);机体维生素A(VA)、维生素E(VE)和β-胡萝卜素(β-C)水平。两组人群应用食物频率表收集调查对象消费的食物种类,做出相应的膳食多样化评分(DDS),分析并评价其日常膳食摄入状况。 结果病例组研究对象的BMI为19.13kg/m2,明显低于对照组人群21.95kg/m2(p0.05),而病例组Hb为128.36g/L,对照组为131.30g/L,但未观察到显著性差异(p0.05);血脂水平分析显示病例组TG、TCHO和HDL含量分别为1.54mmol/L、4.47mmol/L和1.21mmol/L,均低于对照组相应的含量(1.63mmol/L、5.20mmol/L、1.30mmol/L)(p值均0.05);抗氧化酶水平比较显示病例组的SOD和CAT水平分别为78.20U/ml和5.24U/m1,对照组水平分别为83.27U/m1和9.99U/ml,病例组明显低于对照组(p0.05);维生素水平分析显示病例组的VA和VE含量0.256μg/ml和0.385μg/ml,均显著低于对照组的0.385μg/ml和1.182μg/ml (p0.05),但病例组人群的β-C含量为0.048μg/ml,略低于对照组的0.051μg/ml (p0.05)。膳食多样化评分(DDS)结果显示,病例组人群DDS评分为4.81,对照组人群DDS评分为5.21。DDS评分比例病例组以4分最高,占病例组人群的25.7%;对照组以5分最高,占对照组人群的58.6%。DDS评分能够达到7分及7分以上的比例占病例组和对照组分别为15.7%和17.1%。膳食摄入分布情况分析显示病例组人群摄入谷类、肉类、蔬菜类、水果类和豆类食物显著低于对照组人群(p0.05)。 结论肺结核患者的氧化应激水平升高,患者体内抗氧化酶和VA、VE、β-C水平均显著低于正常人群,而且膳食摄入不均衡,谷类、肉类、蔬菜类、水果类和豆类食物摄入较正常人群少。建议积极指导患者改善膳食结构,加强对维生素的补充,防止各种营养素缺乏。
[Abstract]:Objective to investigate the level of oxidative stress and vitamin A E and 尾 -carotene nutrition in pulmonary tuberculosis patients in a certain area, and to investigate the status of daily dietary intake of pulmonary tuberculosis patients in order to guide the reasonable diet of patients. To improve the malnutrition of pulmonary tuberculosis patients, improve the body disease resistance and therapeutic effect to provide scientific basis. Methods A total of 70 pulmonary tuberculosis patients in 2010 were randomly selected as the case group, and 70 healthy people in the same period were matched as the control group. The related nutritional indexes were detected in both groups. These include basic nutritional status, such as body mass index (BMI), hemoglobin (HB), lipid level analysis, such as triglyceride (TGN), total cholesterol (TCHO), high density lipoprotein (HDL), antioxidant enzymes, These include superoxide dismutase (SOD) and catalase (catalase), vitamin A, vitamin E (VEV) and 尾 -carotene (尾 -C.) levels. To analyze and evaluate the status of daily dietary intake by making the corresponding dietary diversity score (DDS). Results the BMI of the case group was 19.13 kg / m ~ (2), which was significantly lower than that of the control group (21.95 kg / m ~ (2) p _ (0.05)), while the HB of the case group was 128.36 g / L and the control group was 131.30 g / L, but there was no significant difference between the two groups (p 0.05). The blood lipid level analysis showed that the levels of TGTCHO and HDL were 1.54 mmol / L 4.47 mmol / L and 1.21 mmol / L, respectively. Compared with the control group, the levels of SOD and CAT were 78.20U/ml and 5.24U / ml, respectively, and those of the control group were 83.27U/m1 and 9.99U / ml, respectively, which were significantly lower than those of the control group (p0.05), vitamin level analysis showed that the level of antioxidant enzyme was lower than that of the control group (P < 0.05). The level of antioxidant enzyme in the case group was significantly lower than that in the control group (P < 0.05), and the levels of SOD and CAT were 5.24U / ml and 9.99U / ml, respectively, which were significantly lower than those in the control group. VA and VE contents of 0.256 渭 g/ml and 0.385 渭 g / ml in the case group were significantly lower than those in the control group (0.385 渭 g/ml and 1.182 渭 g/ml / ml), but the 尾 -C content in the case group was 0.048 渭 g / ml, which was slightly lower than that in the control group (0.051 渭 g/ml / ml, p 0.05). The DDS score of the case group was 4.81, the DDS score of the control group was 5.21.DDS score, the proportion of the case group was the highest, accounting for 25.7% of the case group, and the control group had the highest score of 5 points. The 58.6%.DDS scores of the control group were 15.7% and 17.1% respectively. The distribution of dietary intake showed that the population of the case group ate cereals, meat and vegetables. Fruits and legumes were significantly lower than that of control group (P 0.05). Conclusion the level of oxidative stress in patients with pulmonary tuberculosis is increased, the levels of antioxidant enzymes, VAtoVEand 尾 -C in patients with pulmonary tuberculosis are significantly lower than those in the normal population, and the dietary intake is not balanced, cereals, meat, vegetables, etc. The food intake of fruits and legumes is less than that of normal people. It is suggested that the patients should be actively instructed to improve their dietary structure, to strengthen the supplement of vitamins and to prevent the deficiency of all kinds of nutrients.
