慢性阻塞性肺疾病患者血清激活素A、转化生长因子β1水平及意义

【摘要】:目的:探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者治疗前后血清中激活素A(ACTA)与转化生长因子-β1(TGF-β1)水平,并分析二者的相关性,以及二者与FEV1%的相关性,为COPD患者的病情评估、疗效观察提供临床指导。 方法:采用酶联免疫吸附试验法(enzyme linked immunosorbent assay,ELISA)对40例AECOPD患者治疗前后血清ACTA与TGF-β1水平及40例正常对照组血清ACTA与TGF-β1水平进行检测。同时患者入院时用肺功能仪测定肺功能。 结果:1、COPD组治疗前血清ACTA及血清TGF-β1水平均高于正常对照组,差异均有统计学意义(P0.01);治疗后血清ACTA及血清TGF-β1水平较治疗前均明显降低,,差异均有统计学意义(P0.01,P0.05);COPD治疗后ACTA、TGF-β1浓度均较正常组高,但差异无统计学意义(P>0.05)。2、COPD极重度组ACTA(32.54±3.92)ng/ml高于重度组(31.84±2.78)ng/ml及轻中度组(30.90±4.93)ng/ml,任意两组间比较差异无统计学意义(P>0.05);极重度组TGF-β1(724.29±93.26)ng/L高于重度组(711.91±114.37)ng/L及轻中度组(615.65±191.14)ng/L,任意两组间比较差异无统计学意义(P>0.05)。3、COPD组在治疗前后ACTA与TGF-β1均有正相关性,治疗前的相关系数大于治疗后相关系数;治疗前ACTA、TGF-β1与FEV1%均有负相关性。 结论:(1)血清ACTA与TGF-β1水平在AECOPD显著升高,提示二者可能与机体的急性炎症反应有关,可作为反映COPD急性发作的因子。(2)血清ACTA与TGF-β1水平在AECOPD患者治疗后显著低于治疗前,具有良好的正相关性,提示二者可作为评价治疗效果的敏感指标。(3)血清ACTA、TGF-β1水平在AECOPD患者治疗前明显升高,且与FEV1%呈负相关,表明COPD患者血清ACTA、TGF-β1水平升高与气道炎症和气道重塑存在关联,可间接反映患者通气功能,提示ACTA、TGF-β1参与了COPD的病理生理过程。
[Abstract]:Objective: to investigate the serum levels of activin A (ACTAA) and transforming growth factor- 尾 1 TGF- 尾 1 (TGF- 尾 1) in patients with acute exacerbation of chronic obstructive pulmonary disease (COPD) before and after treatment, and to analyze the correlation between them and FEV1%, so as to evaluate the condition of patients with chronic obstructive pulmonary disease (COPD).Observation of curative effect provides clinical guidance.Methods: the serum levels of ACTA and TGF- 尾 1 in 40 patients with AECOPD before and after treatment were determined by enzyme linked immunosorbent assaysa assay and serum ACTA and TGF- 尾 1 levels in 40 normal controls before and after treatment.At the same time, the pulmonary function was measured by pulmonary function instrument on admission.Results the levels of serum ACTA and TGF- 尾 1 in the control group were significantly higher than those in the control group before and after treatment, and the levels of serum ACTA and TGF- 尾 1 were significantly decreased after treatment.The levels of TGF- 尾 1 in patients with COPD after treatment were significantly higher than those in normal controls.浣嗗樊寮傛棤缁熻瀛︽剰涔
