心室内径比合并生物学指标在COPD合并PH评估中的临床价值

【摘要】:目的:通过回顾性分析来探讨心室内径比与COPD患者合并PH的相关性,并检测CRP、BNP、血气分析、RDW等指标与其相关性,从而更快速便捷地评估COPD合并PH,从而更好地干预治疗、改善预后和增加生存率。方法:本研究收集2015年3月份到2016年12月份期间入住皖南医学院第一附属医院呼吸内科住院治疗的80位COPD患者,通过超声心动图筛选COPD合并PH并观察其心室比(RV/LV)、BNP、血气分析、CRP、D-D、RDW等与PH的相关性。结果:各种指标中,RV/LV、BNP、D-二聚体、CRP、RDW、PCO2在COPD继发PH比COPD单纯组组高,且PH升高中,COPD继发中重度PH较轻度偏高,两两组无统计学差异(F值分别为22.94、20.29、13.51、28.74、17.69、12.73,P均0.01);上述指标经过Pearson相关分析表明与PH具有正相关性(r分别为0.57、0.59、0.46、0.63、0.51、0.48,P均0.01).同时PO2表现为COPD继发PH较COPD患者偏低,COPD继发中重度较轻度组更低,两两组无统计学差异(F=27.63),PO2与PH具有负相关(r=-0.61,P0.01)。结论:RV/LV、BNP、D-二聚体、CRP、RDW、PCO2与PH呈正相关,PO2与PH呈负相关,以上指标在一定程度上可以预测评估PH的严重程度。
[Abstract]:Objective: to investigate the correlation between ventricular diameter ratio and PH in patients with COPD by retrospective analysis, and to detect the correlation between COPD and RDW, so as to evaluate COPD with PH more quickly and conveniently, and to better intervene in treatment.Improve prognosis and increase survival rate.Methods: from March 2015 to December 2016, 80 COPD patients admitted to Department of Respiratory Medicine, the first affiliated Hospital of Southern Anhui Medical College, were collected.COPD combined with PH was screened by echocardiography and its ventricular ratio of RV / LVN was observed. Blood gas analysis was used to analyze the correlation between RV / RDW and PH.Results: the secondary PH of COPD group was higher than that of COPD group, and the secondary PH of RV / LVN BNP-dimer group was higher than that of COPD group, and the secondary PH of RV / LVN BNP-dimer was higher than that of mild PH.At the same time, PO2 showed that the secondary PH of COPD was lower than that of COPD. The secondary PH of COPD was lower than that of mild group. There was no statistical difference between the two groups.Conclusion there is a positive correlation between RDWCO _ 2 and PH and a negative correlation between PH and PCO _ 2. The above indexes can predict the severity of PH to some extent.
