【摘要】：目的：统计2012年本地区社区获得性肺炎（CAP）住院患者中致病菌的构成比，，常见致病菌的耐药情况，为经验治疗提供依据。方法：以回顾性分析研究方法，调阅福州总医院2012年所有确诊为社区获得性肺炎，入院48小时内行纤支镜检查的患者。收集患者的基本资料、细菌种类、药敏结果进行统计分析。结果：确诊肺炎患者209例，其中男性157例，女性52例，年龄16~92岁，需住院的肺炎患者发病年龄段以50~80岁最为多见，吸烟者112例。革兰氏染色检出率为45.5%，阴性菌34%，阳性菌10.5%，真菌1%，细菌培养检出率为34.9%，肺炎克雷伯杆菌16例，铜绿假单胞菌11例，大肠埃希菌8例，真菌（白色念珠菌）7例，肺炎链球菌3例，流感嗜血杆菌1例，溶血葡萄球菌6例。主要革兰氏阴性菌耐药率最高和最低的分别是氨苄西林92.31%，哌拉西林/他唑巴坦15.38%。结论：BALF革兰氏染色及细菌培养检出率分别为45.5%和34.9%，较其他文献报告的痰培养检出率高，但仍有大部分未检出。伴有基础疾病的CAP致病菌以阴性菌为主，肺炎链球菌、流感嗜血杆菌、卡他莫拉菌为苛养菌，较难培养。
[Abstract]:Objective: to analyze the proportion of pathogenic bacteria and drug resistance of common pathogens in the inpatients with community acquired pneumonia (CAP) in our area in 2012, and to provide evidence for empirical treatment.Methods: all the patients diagnosed as community-acquired pneumonia in Fuzhou General Hospital in 2012 were examined with fiberoptic bronchoscopy within 48 hours after admission.Collect the basic data of patients, bacteria types, drug sensitivity results for statistical analysis.Results: 209 cases of pneumonia were diagnosed, among them 157 cases were male, 52 cases were female, the age of pneumonia was 16 ~ 92 years old. The most common age of pneumonia patients in hospital was 50 ~ 80 years old, 112 cases of smokers.One case was Haemophilus influenzae and 6 cases were Staphylococcus haemolyticus.The highest and lowest resistance rates of major gram-negative bacteria were ampicillin 92.31 and piperacillin / tazobactam 15.38 respectively.Conclusion the detectable rates of Gram-BALF Gram staining and bacterial culture were 45.5% and 34.9% respectively, which were higher than those reported in other literatures, but most of them were not detected.The main pathogenic bacteria of CAP with basic diseases were negative bacteria, Streptococcus pneumoniae, Haemophilus influenzae and Cartamora were caustic bacteria, which were difficult to culture.
