单步经皮旋转气管切开术在呼吸衰竭患者抢救中的应用观察

【摘要】：目的观察单步经皮旋转气管切开术(SSPRDT)在呼吸衰竭患者抢救中的应用效果。方法因呼吸衰竭行经皮气管切开术的重症患者78例,其中采用SSPRDT 36例(观察组),采用经皮扩张气管切开术(PDT)42例(对照组)。比较两组手术时间、术中出血量、术前与术中氧分压差、术后并发症发生率。结果观察组顺利完成手术,对照组1例发生大出血,术中改行传统气管切开。观察组手术时间为(7.9±1.34)min、术中出血量为(4.1±1.32)m L、术前与术中动脉血氧分压差为(34.4±7.45)mm Hg、术后并发症发生率为11.1%(4/36),对照组分别为(10.8±1.30)min、(8.8±2.74)m L、(82.1±16.92)mm Hg、28.57%(12/42),两组各项指标相比,P均0.05。对照组3例发生气管后壁损伤或穿孔,观察组未出现此类并发症。结论呼吸衰竭患者抢救中采用SSPRDT操作时间短,置管成功率高,术后并发症少。
[Abstract]:Objective to observe the effect of single step percutaneous rotary tracheotomy (SSPRDTT) in the treatment of respiratory failure. Methods Seventy-eight patients with respiratory failure underwent percutaneous tracheotomy, 36 of them were treated with SSPRDT (observation group) and 42 patients were treated with percutaneous dilatation tracheotomy (control group). The time of operation, the amount of blood loss, the difference of oxygen partial pressure before and during operation, and the incidence of postoperative complications were compared between the two groups. Results in the observation group, the operation was completed successfully, while in the control group, the massive hemorrhage occurred in 1 case, and the traditional tracheotomy was performed during the operation. In the observation group, the operative time was 7.9 卤1.34 minutes, the intraoperative bleeding volume was 4.1 卤1.32mL, the difference between preoperative and intraoperative arterial oxygen partial pressure was 34.4 卤7.45)mm Hg, the postoperative complication rate was 11.1g / 36, and the control group was 10.8 卤1.30ml / min, 8.8 卤2.74ml / L and 82.1 卤16.92)mm Hg 28.57m / L, respectively (P < 0.05). There were 3 cases in the control group with injury or perforation of the posterior wall of trachea, but no such complications occurred in the observation group. Conclusion the operation time of SSPRDT is short, the success rate of catheterization is high, and the postoperative complications are less in patients with respiratory failure.
