慢性阻塞性肺疾病患者外周血单个核细胞NOD2mRNA的表达及意义

【摘要】:目的:本研究通过对COPD患者外周血单个核细胞核内NOD2m RNA的表达及血清中IL-6浓度的测定,根据对不同分组患者外周血单个核细胞核内NOD2 m RNA的表达及血清中IL-6浓度与肺功能的相关性进行研究,探讨NOD2在COPD患者炎症机制中的作用及临床意义。方法:川北医学院附属医院呼吸内科2013年10月至2014年4月门诊及住院缓解后的COPD患者为研究对象。根据其肺功能(FEV1/FVC%、FEV1占预计值%)、症状(m MRC呼吸困难指数)、过去1年急性加重次数进行综合评估,将患者分为A、B、C、D四组,每组30人。同期门诊健康体检者30例作为健康对照组。收集各组患者及对照组外周血标本,用实时荧光定量聚合酶链反应(PCR)测定标本外周血单个核细胞核内NOD2 m RNA的表达,用ELISA法测定标本中的IL-6的浓度。对比分析不同分组COPD患者NOD2 m RNA表达和IL-6浓度的变化,探讨COPD患者NOD2 m RNA表达和IL-6的浓度与COPD分组严重程度的关系及二者的相关性。结果①COPD稳定期组患者及健康对照组的外周血单个核细胞核内NOD2 m RNA分别为0.31±0.18、0.10±0.07,COPD稳定期组明显高于健康对照组,差异有统计学意义(P0.01);COPD稳定期患者及健康对照组的血清IL-6的浓度分别为10.62±5.01pg/ml、3.00±0.40pg/ml,COPD稳定期组明显高于健康对照组,差异有统计学意义(P0.01)。②COPD患者A、B、C、D各组及健康对照组中外周血单个核细胞核内NOD2 m RNA分别为0.18±0.10、0.18±0.08、0.36±0.11、0.51±0.17、0.10±0.07,A、B、C、D组与健康对照组比较,差异均具有统计学意义(P0.01)。COPD患者A、B、C、D各组之间两两比较,D组NOD2 m RNA表达明显高于C组,差异有统计学意义(P0.01),C组NOD2 m RNA表达明显高于A组和B组,差异有统计学意义(P0.01),A组和B组之间差异无统计学意义(P0.05)。③COPD患者A、B、C、D各组及健康对照组血清IL-6的浓度分别为6.36±1.82pg/ml、6.52±1.97pg/ml、12.12±1.78pg/ml、17.50±2.22pg/ml、3.00±0.40pg/ml,A、B、C、D组与健康对照组比较,差异均具有统计学意义(P0.01)。COPD患者A、B、C、D各组之间两两比较,D组IL-6浓度表达明显高于C组,差异有统计学意义(P0.01),C组IL-6浓度明显高于A组和B组,差异有统计学意义(P0.01),A组和B组之间差异无统计学意义(P0.05)。④COPD各组患者外周血单个核细胞核内NOD2 m RNA表达随血清IL-6浓度增加而增加,呈正相关,相关系数r=0.82,相关性有统计学意义(P0.01)。COPD稳定期患者NOD2 m RNA表达与FEV1占预计值%呈负相关,相关系数r=-0.85,相关性有统计学意义(P0.01)。COPD稳定期患者IL-6浓度与FEV1占预计值%呈负相关,相关系数r=-0.74,相关性有统计学意义(P0.01)。结论①COPD患者NOD2受体NOD2 m RNA表达与IL-6浓度较健康对照组明显增高,推测NOD2与IL-6参与了COPD的发生、发展。②随着COPD病情加重,NOD2 m RNA表达与IL-6浓度逐渐升高,NOD2 m RNA与IL-6可能成为COPD病情严重程度评估的指标。③COPD各组中NOD2 m RNA表达与IL-6浓度呈正相关,两者可能相互作用促进炎症反应。
[Abstract]:Objective: To investigate the expression of NOD2m RNA in the nucleus of peripheral blood mononuclear in COPD patients and the determination of IL-6 concentration in serum, and to explore the role of NOD2 in the inflammatory mechanism of COPD patients according to the expression of NOD2 m RNA in the peripheral mononuclear nuclei of peripheral blood and the correlation between the serum IL-6 concentration and the pulmonary function in the peripheral blood mononuclear nuclei of different groups of patients. Methods: the COPD patients in the Department of respiratory medicine of Affiliated Hospital of Chuanbei Medical College from October 2013 to April 2014 were studied. According to the pulmonary function (FEV1/FVC%, FEV1 accounted for estimated%), the symptoms (m MRC respiratory difficulty index), the acute aggravation times of the past 1 years were evaluated comprehensively, and the patients were divided into groups of A, B, C, D four groups. 30 people in each group. 30 cases of health check in the same period were used as the healthy control group. The peripheral blood samples of each group and the control group were collected. The expression of NOD2 m RNA in the nucleus of peripheral blood mononuclear cells was measured by real-time fluorescence quantitative polymerase chain reaction (PCR), and the concentration of IL-6 in the specimens was measured by ELISA. The NOD2 of different groups of COPD patients was compared and analyzed. The expression of M RNA and the change of IL-6 concentration were used to investigate the relationship between the NOD2 m RNA expression and the concentration of IL-6 and the severity of COPD group and the correlation between the two. Results (1) the NOD2 m in the peripheral blood mononuclear nuclei of the COPD stable group and the healthy control group was 0.31 + 0.07, respectively, and the stability group was significantly higher than that of the healthy pair. The difference was statistically significant (P0.01). The concentration of serum IL-6 in patients with COPD stable period and healthy control group was 10.62 + 5.01pg/ml, 3 + 0.40pg/ml, and COPD stable period was significantly higher than that in healthy control group (P0.01). (P0.01), A, B, C, D groups and healthy controls. RNA was 0.18 + 0.10,0.18 + 0.08,0.36 + 0.11,0.51 + 0.17,0.10 + 0.07 respectively, A, B, C, and D group compared with the healthy control group, the difference was significantly higher than that between the 22 groups. There was no statistical significance (P0.01), and there was no significant difference between group A and B group (P0.05). (3) the serum IL-6 concentration of A, B, C, D and healthy control group was 6.36 + 1.82pg/ml, 6.52 + 1.97pg/ml, 12.12 +, 17.50 +, 3 +. 0.01) compared with A, B, C, D, the expression of IL-6 concentration in D group was significantly higher than that of C group, and the difference was statistically significant (P0.01). The IL-6 concentration in the C group was significantly higher than that of the A group and the group, the difference was statistically significant. There was a positive correlation with the increase of serum IL-6 concentration, and the correlation coefficient was r=0.82, the correlation was statistically significant (P0.01), the expression of NOD2 m RNA in.COPD stable patients was negatively correlated with the estimated value of FEV1, the correlation coefficient was r=-0.85, and the correlation was statistically significant (P0.01).COPD regular patients were negatively correlated with the expected value%. The correlation was statistically significant (P0.01). Conclusion (1) the NOD2 m RNA expression of NOD2 receptor in COPD patients and IL-6 concentration was significantly higher than that of the healthy control group. It was suggested that NOD2 and IL-6 participated in the COPD occurrence and development. There was a positive correlation between the expression of NOD2 m RNA and the IL-6 concentration in COPD groups, which may interact to promote inflammatory response.
