维生素D受体(VDR-rs731236、rs2228570)基因多态性与结核易感性的相关研究

【摘要】:目的:采用病例对照研究方法,探讨维生素D受体(VDR)基因多态性与宁夏回族自治区人群肺结核病易感性之间的关系。 方法:病例组来自宁夏回族自治区,2010年经各医院专家按照WHO肺结核病诊断标准确诊的肺结核患者,以与病例组性别相同、民族相同、年龄相当、居住环境相匹配的确认无肺结核病者为对照组。病例对照组采集2-5mL静脉血、肺结核相关的环境因素进行问卷调查。采用聚合酶链式反应-限制性片段长度多态性分析(PCR-RFLP)检测VDR-rs731236、rs2228570位点基因多态性。采用SPSS16.0进行数据统计,,用拟合优度χ2检验进行Hardy-Weiberg平衡检测,基因型、等位基因分布采用χ2检验,肺结核环境因素及其与基因多态性的相互作用采用单因素分析或多因素Logistic回归分析,以P<0.05表示差异具有显著性意义。 结果:对病例组993例,对照组880例进行VDR基因多态性分型结果,两位点均符合Hardy-Weiberg平衡。PCR-RFLP结果的多因素Logistic回归分析显示,VDR-(TT+Tt)、-(FF+ff)基因型在病例对照组中的分布差异有统计学意义(P<0.05);男性VDR-TT基因型、回族VDR-(FF+ff)基因型在病例对照组中的分布具有差异显著性。结论:VDR基因多态性与宁夏回人群肺结核易感性有关。VDR-(TT+Tt)、男性 VDR-TT基因型可能是肺结核病的保护因素;VDR-(FF+ff)、回族VDR-(FF+ff)基因型可能是肺结核发病的危险因素。
[Abstract]:Objective: To explore the relationship between vitamin D receptor (VDR) gene polymorphism and susceptibility to pulmonary tuberculosis in the Ningxia Hui Autonomous Region population by case-control study.
Methods: the case group was from the Ningxia Hui Autonomous Region. In 2010, the pulmonary tuberculosis patients were confirmed by the WHO pulmonary tuberculosis diagnosis standard by all the experts in the hospital. The patients with the same sex, the same nationality, the same age, the living environment matched the patients with no pulmonary tuberculosis as the control group. The case control group collected 2-5mL venous blood and pulmonary tuberculosis. The environmental factors were investigated by a questionnaire. Polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) was used to detect the polymorphism of VDR-rs731236 and rs2228570 loci. SPSS16.0 was used for data statistics, Hardy-Weiberg balance test was performed with the goodness of fit chi 2 test. The genotype, allele distribution was tested by chi 2 test, pulmonary tuberculosis The interaction between environmental factors and genetic polymorphisms was analyzed by univariate analysis or multivariate Logistic regression analysis. The difference was significant in P < 0.05.
Results: 993 cases in the case group and 880 cases in the control group were the results of the polymorphism of VDR gene, and the multiple factor Logistic regression analysis of the two loci was consistent with the Hardy-Weiberg balance.PCR-RFLP results. The distribution of VDR- (TT+Tt) and (FF+ff) genotype in the case control group was statistically significant (P < 0.05); the male VDR-TT genotype, the VDR- of the Hui nationality was VDR-. (FF+ff) genotype distribution in the case-control group was significantly different. Conclusion: VDR gene polymorphism is associated with susceptibility to tuberculosis in Ningxia population.VDR- (TT+Tt), male
VDR-TT genotype may be a protective factor for pulmonary tuberculosis; VDR- (FF+ff) and Hui VDR- (FF+ff) genotype may be a risk factor for pulmonary tuberculosis.
