阻塞性睡眠呼吸暂停低通气综合征与P波离散度关系的研究

【摘要】:目的:探讨P波离散度与OSAHS严重程度之间的关系,并进一步阐明用P波离散度衡量OSAHS疾病严重程度的可行性,分析OSAHS导致P波离散度发生变化的可能机制。方法:通过12导联心电图机,进行P波离散度的定量测量,并以多导睡眠监测,测量不同严重程度OSAHS患者、正常成人呼吸暂停低通气指数(AHI)、最低血氧饱和度(LSaO2)、平均血氧饱和度(MSaO2)的水平,,比较各组间及不同严重程度的OSAHS患者P波离散度的水平差异,同时探讨其与血氧饱和度的关系。结果:OSAHS患者的P波离散度较正常对照组有明显的升高,(46.09±13.40ms vs34.10±10.75ms,P0.0001)。而且不同程度OSAHS三组P波离散度两两比较,P波离散度与OSAHS严重程度,或者说AHI指数成正相关(r=0.365,P=0.003)。同时,最低氧饱和度与P波离散度呈负相关。(r=-0.498,P0.0001)结论:这项研究表明,P波离散度的大小与OSASH患者的严重程度呈正相关。心房扩张,低氧血症可能通过心房结构和电的变化导致P波离散度的升高,同时也可以利用P波离散度初筛、评价OSASH,预测未来发生房颤的风险。
[Abstract]:Objective: to explore the relationship between P-wave dispersion and OSAHS severity, and to further clarify the feasibility of using P-wave dispersion to measure the severity of OSAHS disease, and to analyze the possible mechanism of P-wave dispersion change caused by OSAHS. Methods: P-wave dispersion was measured quantitatively by 12-lead electrocardiograph and polysomnography was used to measure OSAHS patients with different severity. The levels of apnea hypopnea index (AHI), lowest oxygen saturation (LSaO2) and mean oxygen saturation (MSaO2) in normal adults were compared. Results the P wave dispersion in the patients with OSAHS was significantly higher than that in the control group (46.09 卤10.75 Ms / P 0.0001). In addition, P wave dispersion in three groups of OSAHS with different degrees was positively correlated with the severity of OSAHS, or AHI exponent (r = 0.365 P0. 003). At the same time, there was a negative correlation between the minimum oxygen saturation and P wave dispersion. Conclusion: this study shows that the size of P wave dispersion is positively correlated with the severity of OSASH patients. Atrial dilation and hypoxemia may lead to the increase of P wave dispersion through changes in atrial structure and electricity. At the same time, we can use P wave dispersion to evaluate OSASHs and predict the risk of atrial fibrillation in the future.
