放射性肺炎合并肺部感染临床观察与治疗

【摘要】：目的分析中西药结合联合对症护理治疗放射性肺炎合并肺部感染的疗效。方法采用随机数字表法将2010年3月-2013年2月80例放射性肺炎合并肺部感染患者分为两组,对照组38例,予以常规抗感染、激素、补液支持治疗,同时予以呼吸科常规护理;试验组42例,在常规西医治疗和常规护理的基础上,加以清肺化痰祛瘀法治疗和放射性肺炎对症护理,对比3周后两组患者的身体功能状态(KPS)评分和肺部CT肺实变病灶大小,同时对其临床症状进行评价。结果两组治疗后的KPS评分明显高于治疗前,肺部CT肺实变病灶明显小于治疗前(P0.05);治疗后试验组的KPS评分明显高于对照组,肺部CT肺实变病灶明显小于对照组,试验组治疗后缓解38例占90.48%,未缓解4例占9.52%,对照组治疗后缓解24例占63.16%,未缓解14例占36.84%,试验组临床疗效明显优于对照组(P0.05)。结论中西药结合联合对症护理治疗对放射性肺炎合并肺部感染有明显的疗效,值得临床进一步探讨与推广。
[Abstract]:Objective to analyze the curative effect of TCM combined with western medicine combined with symptomatic nursing in the treatment of radiation pneumonia complicated with pulmonary infection. Methods from March 2010 to February 2013, 80 patients with radiation pneumonia complicated with pulmonary infection were randomly divided into two groups: control group (n = 38), routine anti-infection therapy, hormone and fluid resuscitation support therapy, and routine nursing care in respiratory department. On the basis of routine western medicine treatment and routine nursing, 42 patients in the experimental group were treated with clearing the lung and removing phlegm and removing blood stasis and with the symptomatic nursing of radiation pneumonia. The KPS score of the two groups and the size of the pulmonary CT pulmonary consolidation lesion were compared after 3 weeks. At the same time, the clinical symptoms were evaluated. Results the KPS score of the two groups after treatment was significantly higher than that before treatment, the pulmonary CT pulmonary consolidation lesion was significantly smaller than that before treatment (P0.05), the KPS score of the experimental group after treatment was significantly higher than that of the control group, and the pulmonary CT pulmonary consolidation lesion was significantly smaller than that of the control group. There were 38 cases of remission after treatment in the trial group (90.48%), 4 cases of non-remission (9.52%), 24 cases of relief (63.16 cases) and 14 cases (36.84%) of the control group after treatment. The clinical effect of the trial group was significantly better than that of the control group (P0.05). Conclusion the combination of traditional Chinese medicine and western medicine combined with symptomatic nursing therapy has obvious curative effect on radiation pneumonia complicated with pulmonary infection, which is worthy of further clinical discussion and popularization.
