清肺解毒扶正汤对老年肺炎患者C3、免疫球蛋白的影响及临床研究

【摘要】：目的：客观评价清肺解毒扶正汤对老年肺炎患者的临床疗效及对C3、免疫球蛋白的影响，并对其安全性加以评估。方法：老年肺炎（气阴两虚兼痰热壅肺型）共纳入72例。用随机数字表法，分为治疗组和对照组各36例，观察过程中剔除脱落6例，最终治疗组34例，对照组32例。两组均采取平行对照设计，治疗组在对照组综合治疗基础上加服清肺解毒扶正汤，观察两组治疗前后患者的临床症状、体征、相关指标，并对结果进行统计分析。结果：⑴两组患者总疗效、证候疗效、证候积分比较，P＜0.05，说明治疗组均优于对照组；⑵治疗组在治疗咳嗽、痰量、发热、出汗、乏力、纳差、肺部体征的单个症状疗效（P＜0.05）及疗程方面明显优于对照组（P＜0.01）；⑶测定指标：治疗组治疗后异常的C3、IgM、IgA与治疗前比均降至正常范围，与对照组比改善较为明显（P＜0.01）。两组IgG治疗前后无显著性差异（P＞0.05）；⑷治疗组在胸片炎症吸收、异常的白细胞改善以及痰培养方面优于对照组（P＜0.05）；⑸安全性指标和不良反应观察方面，差异无统计学意义（P＞0.05），两组治疗均相对安全。结论：清肺解毒扶正汤不仅可以控制感染，促进炎症吸收，提高病菌清除率，，缩短疗程，提高临床疗效，减少二重感染的发生，还可以调节免疫功能，提高患者免疫力，疗效好于单纯西药治疗。提示该药具有良好的治疗效果且相对安全，有重要的临床应用价值。
[Abstract]:Objective: to evaluate the clinical efficacy of Qingfei jiedu Fuzheng decoction in elderly patients with pneumonia and its effect on C _ 3 and immunoglobulin and to evaluate its safety. Methods: 72 cases of senile pneumonia (qi-yin deficiency and phlegm-heat obstructing lung type) were included. The patients were randomly divided into two groups: treatment group (n = 36) and control group (n = 36). In the course of observation, there were 6 cases of elimination and shedding, 34 cases of final treatment group and 32 cases of control group. The treatment group was treated with Qingfei jiedu Fuzheng decoction on the basis of comprehensive treatment in the control group. The clinical symptoms, signs and related indexes were observed before and after treatment, and the results were statistically analyzed. Results the total curative effect and syndromes score of the two groups were lower than those of the control group (P < 0.05), which indicated that the treatment group was superior to the control group in the treatment of cough, sputum volume, fever, sweating, fatigue and anorexia. The curative effect of single symptom of lung sign (P < 0.05) and the course of treatment were significantly better than that of control group (P < 0.01). After treatment, the abnormal C _ 3 IgM _ 3 IgA in the treatment group was decreased to the normal range compared with the control group (P < 0.01), and the improvement was more obvious than that in the control group (P < 0.01). There was no significant difference between the two groups before and after IgG treatment (P > 0. 05). The treatment group was superior to the control group in terms of chest film inflammation absorption, abnormal leukocyte improvement and sputum culture (P < 0. 05). There was no significant difference between the two groups (P > 0.05). Conclusion: Qingfei jiedu Fuzheng decoction can not only control infection, promote inflammation absorption, improve bacterial clearance rate, shorten course of treatment, improve clinical efficacy, reduce the occurrence of double infection, but also regulate immune function and improve patients' immunity. The curative effect is better than that of western medicine alone. It suggests that the drug has good therapeutic effect and is relatively safe and has important clinical application value.
