幽门螺杆菌感染与慢性阻塞性肺疾病关系的研究

【摘要】:目的:通过分析COPD患者血清细胞因子IL-6、IL-17、anti-Hp-IgG的变化,及与肺功能FEV1%预计值的相关性,探讨幽门螺杆菌(Hp)感染与慢性阻塞性肺疾病(COPD)发病的相关性及可能作用机制。 方法:根据GOLD指南,通过临床表现、肺功能测定确诊COPD,使用支气管扩张剂后FEV1/FVC0.7,确定为持续性气流受限从而诊断COPD,选取31例(男20例,女11例,平均年龄62.74±4.27)2011.12-2012.7经我院门诊检查临床确诊的COPD稳定期患者为COPD组,同时随机选取2012年7月于我院体检中心体检性别、年龄相匹配的健康志愿者30例(男18例,女12例,平均年龄61.97±3.16)为对照组。所有入选者均进行肺功能检测,并于肺功能检查的次日空腹抽取外周静脉血5ml,分离血清,-70℃超低温冰箱冷冻,统一采用ELISA方法检测血清细胞因子IL-6、IL-17的水平和anti-Hp-IgG水平。 结果: (1)COPD组血清anti-Hp-IgG水平明显高于对照组(P=0.042),且患者肺功能FEV1%预计值与anti-Hp-IgG水平呈负相关(r=-0.662,P0.001); (2)COPD组血清IL-6、IL-17水平分别明显高于对照组(P0.001,P=0.036), (3)COPD组IL-17水平与anti-Hp-IgG水平呈正相关(r=0.671,P0.001)而与FEV1%预计值呈负相关(r=-0.615,P0.001)。 (4)COPD组IL-6水平与anti-Hp-IgG水平和肺功能FEV1%预计值没有相关性。 结论:(1)Hp感染刺激血清IL-6、IL-17水平增高。 (2)COPD患者血清anti-Hp-IgG、IL-6、IL-17水平增高,且IL-17水平与anti-Hp-IgG呈正相关而与肺功能FEV1%预计值呈负相关。 (3)Hp感染促进了COPD的进展,两者所启动的体内炎症反应也许是两者相互作用的机制,可能与IL-17激活有关。
[Abstract]:Objective: to investigate the relationship between Helicobacter pylori (Hp) infection and (COPD) in patients with chronic obstructive pulmonary disease (COPD) by analyzing the changes of serum cytokine IL-6,IL-17,anti-Hp-IgG and its correlation with FEV1% prediction of pulmonary function in patients with COPD. Methods: according to the GOLD guidelines, 31 cases (male 20, female 11) with COPD, were diagnosed as persistent airflow limitation after COPD, was confirmed by pulmonary function test after bronchiectasis. The stable COPD patients diagnosed by outpatient examination in our hospital from 12 to 2012.7 were selected as COPD group. 30 healthy volunteers (18 males and 12 females) who were examined in the physical examination center of our hospital in July 2012 were selected randomly. All the patients were tested for pulmonary function, and the peripheral venous blood was extracted from 5 ml on an empty stomach the next day after pulmonary function examination, and the serum was isolated and frozen in ultra-low temperature refrigerator at 70 鈩,
