文章摘要:目的对安徽医科大学第一附属医院近5年住院患者下呼吸道感染细菌菌谱及耐药性进行调查,指导临床合理使用抗菌药物。方法对安徽医科大学第一附属医院2009年1月-2013年12月住院患者痰液细菌培养标本进行回顾性分析,本次研究,数据录入采用WHONET5.6软件,数据分析采用SPSS17.0软件。结果共收集下呼吸道痰液标本37410例,细菌培养阳性结果6564例,其中革兰氏阳性菌和革兰氏阴性菌分别是617株(9.4%)和5947株(90.6%)。鲍曼不动杆菌和铜绿假单胞菌分别位于细菌检出率的前两位,呈逐年增长趋势,其次是肺炎克雷伯菌,年检出率呈递减趋势。耐药性分析结果显示,,铜绿假单胞菌对阿米卡星和四代头孢菌素的耐药率低,多在20.0%以下。鲍曼不动杆菌对头孢哌酮/舒巴坦和米诺环素的耐药率较其他药物低。金黄色葡萄球菌耐药率高,未发现耐万古霉素、替考拉宁菌株。结论通过下呼吸道感染耐药菌的监测,有助于了解耐药菌的分布及对抗生素的耐药情况,指导临床医生合理使用抗生素,提高临床治疗效果。
Abstr:Objective To investigate the bacterial spectrum of lower respiratory tract and the drug resistance change in the first affiliated hospital of Anhui Medical University in recent five years, and to provide basis for the reasona- ble use of antibiotics in clinic. Methods A retrospective survey of the hospitalized patients with bacterial culture of spu- tum specimens from January 2009 to December 2013 was conducted. All the data were input WHONET 5.6 and statistically analyzed by SPSS 17.0. Results 6 564 bacteria strains were isolated with bacteria from 37 410 sputum spectimens of low- er respiratory tract, including gram-positive bacteria 617 (9. 4% ) and gram-negative bacteria 5 947 (90. 6% ). The bacteri- a detection rate of acinetobacter baumannii and pseudomonas aeruginosa ranked in the top two, showed increasing trend, followed by klebsiella pneumoniae, whose detection rate presented decreasing trend. The resistance results showed that the resistance rate of pseudomonas aeruginosa against amikaein and the fourth generation cepbalosporins was about 20. 0% or lower. Staphylococcus ureus was resistant to many kinds of anti-biotics, however, no vancomycin and teicoplanin resistant strains were "found yet. Conclusions Through the surveillance of drug-resistant bacteria of lower respiratory tract infec- tions, help to understand the distribution of drug-resistant bacteria and bacterial resistance of antibiotics and guide the ra- tional use of antibiotics in clinic, and to improve the efficacy of treatment.
