N-乙酰半胱氨酸对AECOPD血清生物标志物的影响

【摘要】:目的:探讨N-乙酰半胱氨酸(N-acetylcysteine,NAC)治疗急性加重期慢性阻塞性肺疾病(Acuteexacerbationsofchronicobstructivepulmonarydisease,AECOPD)患者的疗效及其对血清肺表面活性物质蛋白A(Surfactantprotein A)、肺表面活性物质蛋白D(Surfactantprotein D)、Clara细胞蛋白16(Claracellprotein16,CC16)水平的影响及意义。方法:选取2015年8月-2016年5月大连大学附属中山医院住院的肺功能Ⅲ、Ⅳ级AECOPD患者共80例,随机分为对照组和NAC干预组各40例,对照组予以基础治疗,NAC干预组在对照组治疗上联合NAC,疗程为2周。对照组和NAC干预组患者分别于治疗前、治疗2周后行CAT评分、肺功能测定。用酶联免疫吸附法(ELISA法)测定治疗前后血清SP-A、SP-D、CC16的含量,并将上述各指标与肺功能、CAT评分进行相关性分析。结果:1.组内对比显示,NAC干预组治疗后CC16较治疗前升高,SP-A、SP-D均较治疗前降低;对照组治疗后CC16较治疗前升高,SP-A较治疗前降低。上述差异均有统计学意义(P0.05)。治疗后组间对比显示,NAC干预组CC16较对照组升高。SP-D、SP-A均较对照组降低。上述差异均有统计学意义(P0.05)。2.组内对比显示,NAC干预组治疗后FEV1、FEV1%pre、FEV1/FVC(%)较治疗前升高;对照组治疗后FEV1/FVC(%)较治疗前升高,上述差异均有统计学意义(P0.05)。治疗后组间对比显示,NAC干预组中肺功能FEV1、FEV1/FVC(%)均较对照组升高,上述差异均有统计学意义(P0.05)。3.组内对比显示,NAC干预组治疗后CAT评分较治疗前降低,对照组治疗后CAT评分较治疗前降低,差异有统计学意义(P0.05)。治疗后组间对比显示,NAC干预组CAT评分低于对照组,差异有统计学意义(P0.05)。4.两组治疗前各血清生物标志物与肺功能、CAT评分的相关性。SP-D与FEV1%pre、FEVl/FVC(%)呈负相关,与CAT评分呈正相关,差异有统计学意义(P0.05);CCl6与FEVl%pre、FEV1/FVC(%)呈正相关,与CAT评分呈负相关,差异有统计学意义(P0.05);SP-A与FEV1/FVC(%)呈正相关,差异有统计学意义(P0.05)。结论:1.NAC治疗可提高CC16水平,降低SP-A、SP-D水平,提高肺功能,改善临床症状。2.各生物标志物与肺功能及CAT评分有相关性,提示生物标志物对COPD的诊断及评估有一定意义。
