稳定期COPD患者外周血中Th17和Treg的表达及平衡关系变化

【摘要】:目的观察辅助性T细胞17(Th17)和调节性T淋巴细胞(Treg)在稳定期慢性阻塞性肺疾病(COPD)患者外周血中的表达及平衡关系变化,探讨其在COPD发病机制的作用。方法选取22例肺功能正常不吸烟者(A组)、22例肺功能正常吸烟者(B组)以及36例稳定期COPD患者(C组)作为研究对象。运用流式细胞术检测外周血提取外周单个核细胞(PBMC)中的Th17和Treg的表达及平衡关系的变化。结果 C组外周血Th17的表达率明显高于B组及A组,差异有统计学意义(P0.01),B组Th17细胞外周血中的表达高于A组(P0.01);C组外周血中Treg细胞的表达率高于A组(P0.01)和B组(P0.01),B组高于A组,差异有统计学意义(P0.01);Th17/Treg比值C组明显高于A组和B组,A组和C组的差异有统计学意义(P0.01),但A组与B组及B组与C组的差异无统计学意义(P0.05)。3组数据汇总分析外周血中Th17细胞和Treg细胞的表达与吸烟量呈正相关(P0.01),Th17/Treg的比值与吸烟量无明显相关性(P0.05);3组数据分析Th17细胞、Treg细胞以及Th17/Treg比值与第1秒用力呼气容积(FEV1)/预计值(%)和FEV1/用力肺活量(FVC)(%)呈负相关。结论 C组外周血中Th17细胞和Treg细胞表达水平明显升高,可能参与COPD的发病机制;C组的Th17/Treg比值升高,平衡失调,可能是COPD发病的免疫机制;Th17和Treg细胞表达水平与吸烟量呈正相关,与FEV1/预计值(%)及FEV1/FVC(%)呈负相关。
[Abstract]:Objective to observe the expression and balance of helper T cell 17 (Th17) and regulatory T lymphocyte (Treg) in the peripheral blood of patients with chronic obstructive pulmonary disease (COPD) at stable stage, and to explore its role in the pathogenesis of COPD. Methods 22 patients with normal pulmonary function (group A), 22 patients with normal pulmonary function (group B) and 36 patients with stable COPD (group C) were studied. Flow cytometry was used to detect the expression of Th17 and Treg in peripheral blood extracted peripheral mononuclear cells (PBMC). Results the expression rate of Th17 in peripheral blood of group C was significantly higher than that of group B and group A, and the difference was statistically significant (P0.01), B group was higher than group A (P0.01); C group) in Th17 cell expression rate was higher than A group (P0.01); C group) and B group (P0.01), B group was higher than A group). The ratio of Th17/Treg in group C was significantly higher than that in group A and group B (P0.01), but there was no significant difference between group A and group B and group C (P0.05). The expression of Th17 cells and Treg cells was positively correlated with smoking volume (P0.01), while the ratio of Th17/Treg was not significantly correlated with smoking volume (P0.05), and the ratio of Th17 cells, Treg cells and Th17/Treg was negatively correlated with forced expiratory volume (FEV1) / predictive value (%) and forced vital capacity (FVC) (%) of FEV1/. Conclusion the expression of Th17 cells and Treg cells in peripheral blood of group C is significantly increased, which may be involved in the pathogenesis of COPD, and the ratio of Th17/Treg in group C is increased and the balance is not balance. The expression levels of Th17 and Treg cells were positively correlated with smoking and negatively correlated with the predicted value of FEV1/ (%) and FEV1/FVC (%).
