COPD患者不同机械通气参数对腹腔压力的影响

【摘要】:目的探讨机械通气状态下不同参数变化对腹腔压力的影响。方法选取慢性阻塞性肺疾病(COPD)伴呼吸衰竭行机械通气的42例患者,当患者病情稳定后,在同步间歇指令通气(SIMV)+压力支持通气(PSV)的通气模式下,观察吸气平台时间(Tplat)、吸气时间(Ti)、呼气末正压(PEEP)的变化对腹腔压力的影响。结果在SIMV+PSV通气模式下,Tplat、Ti的变化对腹腔压力无明显影响(P0.05)。SIMV+PEEP0与基线值比较差异无统计学意义(P0.05),随着PEEP的增加,腹腔压力随之升高(P0.05)。结论机械通气状态下,呼气末正压的变化将会对腹腔压力产生显著影响。选择合理的机械通气参数至关重要。
[Abstract]:Objective to investigate the effect of different parameters on abdominal pressure during mechanical ventilation. Methods 42 patients with chronic obstructive pulmonary disease (COPD) complicated with respiratory failure underwent mechanical ventilation. When the patients were in stable condition, the patients were ventilated under the condition of synchronous intermittent mandatory ventilation (SIMV) pressure to support the ventilation of (PSV). To observe the effect of positive end expiratory pressure (PEEP) on abdominal pressure during inspiratory plateau time (Tplat),) and inspiratory time (Ti),). Results under SIMV PSV ventilation mode, the change of Tplat,Ti had no significant effect on abdominal pressure (P0.05). SIMV PEEP0 compared with baseline value no significant difference (P0.05), with the increase of PEEP, abdominal pressure increased (P0.05). Conclusion during mechanical ventilation, the change of positive end-expiratory pressure has a significant effect on abdominal pressure. It is very important to select reasonable mechanical ventilation parameters.
