急诊ICU中序贯通气治疗的慢性阻塞性肺疾病急性发作患者撤机时机及影响撤机因素的临床分析

【摘要】:目的探讨急诊ICU(EICU)中序贯通气治疗的慢性阻塞性肺疾病急性发作(AECOPD)患者撤机时机的选择,分析影响机械通气持续时间的因素。方法选取2012年4月至2014年4月中国医科大学附属第一医院EICU收治的经有创-无创序贯通气治疗的AECOPD患者132例,收集患者的一般情况、机械通气时间、浅快呼吸指数(RSBI)、急性生理学和慢性健康状况Ⅱ(APACHEⅡ)评分、自主排痰能力、血气分析、血浆前白蛋白(pre-ALB)水平、脑钠肽(BNP)、降钙素原(PCT)等指标。对各项指标进行单因素分析,并将具有统计学意义的指标纳入Logistic回归分析。结果 132例患者中,96例(72.7%)需要≤7 d的有创呼吸机辅助通气,19例(14.3%)需要7 d的有创呼吸机辅助通气;115例撤机成功的患者中83例(72.1%)首次撤机即成功。对于有自主排痰能力的患者,以RSBI95 bpm/L、APACHEⅡ评分23为指标预测首次撤机成功的敏感度为91.6%,特异度为62.5%。机械通气持续时间与p H、Pa CO2无相关性(P0.05),与血浆pre-ALB水平(OR:0.31,95%CI:0.21~0.63,P=0.03)、BNP(OR:3.43,95%CI:12.33~16.43,P=0.01)、PCT(OR:3.67,95%CI:1.90~2.11,P=0.01)以及APACHEⅡ评分(OR:2.89,95%CI:1.10~2.87,P=0.01)相关。结论在EICU中大多数经有创-无创序贯通气治疗的AECOPD患者基本可以在7 d内完成撤机,综合RSBI、APACHEⅡ评分和患者自主排痰能力有助于撤机时机的选择。
