无创呼吸机在慢性阻塞性肺病合并左心衰竭高龄患者中的应用价值

【摘要】：目的探究无创呼吸机在慢性阻塞性肺病(COPD)合并左心衰竭高龄患者中的应用价值。方法选取80例COPD合并左心衰竭且符合条件的高龄患者,将患者随机分为两组,各40例,对照组患者使用常规药物治疗,观察组采用无创呼吸机加常规药物治疗,观察并记录患者治疗前和治疗后1.5 h、3 h和7 d患者的心率(HR)、血压(BP)、脑钠肽(BNP)、呼吸频率(RR)和动力血气分析(PaCO2、PaO2、pH)。结果两组患者的BNP、PaCO2水平都比治疗前提高(P0.05),PaO2水平较治疗前降低(P0.05),而pH较治疗前都有所升高。观察组的患者PaCO2在治疗后1.5h、3 h和7 d都低于对照组(P0.05),而PaO2和pH治疗后1.5 h、3 h和7 d都高于对照组(P0.05)。两组患者BP、HR和RR治疗后都较治疗前有所降低(P0.05),观察组患者治疗后1.5 h、3 h和7 d的BP、HR和RR都明显低于对照组(P0.05)。结论两组患者的HR、SBP、BNP、RR和动力血气分析(PaCO2、PaO2、pH)治疗后都有所改善,而采用无创呼吸机治疗的患者疗效优于只使用常规药物治疗的患者。
[Abstract]:Objective to investigate the value of noninvasive ventilator in elderly patients with chronic obstructive pulmonary disease (COPD) complicated with left heart failure. Methods 80 elderly patients with COPD complicated with left heart failure were randomly divided into two groups, 40 patients in each group. The patients in the control group were treated with conventional drugs. The patients in the observation group were treated with noninvasive ventilator plus conventional drugs. The heart rate (HR), blood pressure (BP), brain natriuretic peptide (BNP), was observed and recorded before treatment and 1.5 hours, 3 hours and 7 days after treatment. Respiratory frequency (RR) and dynamic blood gas analysis (PaCO2,PaO2,pH). Results the level of BNP,PaCO2 in both groups was higher than that before treatment (P 0.05), the level of PaO2 was lower than that before treatment (P 0.05), and the level of pH was higher than that before treatment. The PaCO2 of the patients in the observation group was lower than that in the control group at 1.5 h, 3 h and 7 d after treatment (P 0.05), while at 1.5 h after PaO2 and pH treatment, it was higher than that in the control group at 3 h and 7 d (P 0.05). BP,HR and RR in both groups were lower than those before treatment (P 0.05). BP,HR and RR at 3 h and 7 d in the observation group were significantly lower than those in the control group at 1.5 h after treatment (P 0.05). Conclusion HR,SBP,BNP,RR and dynamic blood gas analysis (PaCO2,PaO2,pH) in both groups are improved after treatment, and the curative effect of patients treated with noninvasive ventilator is better than that of patients treated with conventional drugs alone.
