尿路真菌感染调查与药敏分析

【摘要】:目的探讨尿路医院感染真菌的分布与对抗真菌药物的敏感性,为临床合理选择抗真菌药物治疗提供指导。方法收集2013年7月-2015年7月医院收治的经中段尿细菌培养确诊为尿路真菌感染的237例患者为研究对象,采集患者尿液标本,检出的菌株采用科玛嘉显色培养基进行鉴定,并通过ATBFUNGUS3真菌药敏试剂进行药物敏感性试验。结果清洁中段尿标本,经真菌培养后分离得到237株真菌,白色假丝酵母菌113株占47.68%,光滑假丝酵母菌79株占33.33%;患者主要集中在综合ICU、泌尿外科、心内科、内分泌科及呼吸内科,以上科室尿路真菌感染占83.12%;白色假丝酵母菌、光滑假丝酵母菌及热带假丝酵母菌对两性霉素B和氟胞嘧啶敏感,耐药率分别为0和0.88%、1.27%和0、0和0,白色假丝酵母菌对氟康唑、伊曲康唑以及伏立康唑的耐药率均10.00%,而后两者均10.00%,克柔假丝酵母菌主要对氟胞嘧啶敏感,对伊曲康唑耐药率38.4%,对伏立康唑的耐药率30.7%。结论白色假丝酵母菌、光滑假丝酵母菌以及热带假丝酵母菌是院内尿路真菌感染的主要真菌,其对氟胞嘧啶以及两性霉素B具有较好的敏感性。
[Abstract]:Objective to investigate the distribution of urinary tract nosocomial infection fungi and the sensitivity of antifungal drugs. Methods from July 2013 to July 2015, 237 cases of urethral fungal infection diagnosed by bacterial culture in middle urinary tract were collected. Object. The urine samples of the patients were collected, and the strains were identified by the Komagar chromogenic medium, and the drug sensitivity tests were carried out by the ATBFUNGUS3 fungal drug sensitive reagent. Results the urine samples were cleaned. 237 strains of fungi were isolated after culture, 113 strains of Candida albicans accounted for 47.68%, 79 strains of Candida smooth accounted for 33.33%. The patients were mainly concentrated in integrated ICU, urology, cardiology, endocrine department and respiratory department. The urinary tract fungal infection accounted for 83.12% in the above departments. Candida albicans, Candida smooth and Candida tropicalis were sensitive to amphotericin B and fluorocytosine, with resistance rates of 0 and 0.88% and 0 and 0, respectively. The resistance rates of Candida albicans to fluconazole itraconazole and voleconazole were 10.00 and 10.00 respectively. The resistance rate to itraconazole was 38.4%, and the resistance rate to voriconazole was 30.7%. Conclusion Candida albicans, Candida smooth and Candida tropicalis are the main fungi of urinary tract fungal infection in hospital. It is sensitive to fluorocytosine and amphotericin B.
